My ideas:
1) Your account was h4x0r'd (hacked).
2) Someone you know had your login info (way back when and/or recently) and chose PuddlesofPink for fun while browsing the forums.
3) YOU chose PuddlesofPink, got embarassed, but don't know how to change it, so you're cleverly asking what happened like you don't really know.
(If you don't think it's any of those, maybe ask a community rep about your forum registration and use? And is your game account still intact character-wise?)

Haha, well yeah those were the three I was thinking. But..
1.) This was the one I immediately thought initially. Oh crap, my account was hacked but after posting my request for help, I saw that it was the first post to my account. If they went through the trouble of hacking me, wouldn't it make sense for them to have made a post? Then again, why hack my forum name?
2.) I never gave my information out but I suppose its possible that someone got it, my account password really is terrible, but again I would have expected more havok from someone who does all of that. How does someone randomly go about something like that anayway?
3.) This is the one that to my knowledge makes the most sense but also seems least likely. I really and truly don't like the color pink. (ha) And I'd liek to think I have a better taste in names. If I did do it way back when, I sure don't remember doing it or know why I would.
I was wondering if it was possible for accounts to be crossed. It was in fact my game info put in so i didn't accidently log into someone elses. As far as character-wise, that all seemed to be accounted for. I used to be quite the alt-monster though so I admit it'd be hard to keep track. Nevertheless, I saw I think all of my characters there. Though yesterday I did notice my characters were lacking any enhancements in their slots, a team member reminded me of respecs which I knew was pretty likely. So all in all, I think that account was undisturbed. Maybe it was 3, idk. Just hoping it wasn't. XD
If your account was hacked, they might never have posted on the forums. Why? If they knew what they were doing was against the EULA (and, you know, laws), then why draw attention to themselves?
Also, if some random gold-farming company hacked your account, they might have only ever needed to use it to shuttle influence back and forth, and never needed the account to log onto the forums. Still, they might have changed things just so that it was tougher for you to get it back.
I would definitely change your user name and password, talk to support, and make sure all of your characters are still there.

Contact support either by telephone or a ticket and see what they can do for you. They may be willing to change it for you, especially since it could sound kinda dirty if you really have a twisted mind(like I do)
You know. Many months ago when they swapped the type of boards that we use, when I logged in, I was into someone elses account. When I logged out and back in, it corrected itself and I was once again Sionwyn.
Can't remember who's account it was but I immediately signed out.
I PMd someone, but don't remember who and I clean my boxes out regularly so no longer have the PM.
A PM is a good idea.

Now, after I saw the name, however, I did exactly what you did and tried to log off and back on, hoping for an error. There was no difference though, I was still named Pink puddle. I'll try PMing someone when I figure out who I should PM.
See, I was going to make a joke about them having too many Cosmopolitans.

I've just returned to the COX games about a week ago and I was hoping to dive into activity and really integrate with the community this time around. None of that is really important though, what is of my concern is that when I tried to register an account just now, it said my actual game account was already in use. At first I wondered if I might have made an account before now and forgot about it but after signing in with that information, I discovered that the email account did not match any of mine and that the user-name "Puddles of Pink" doesn't sound like anything I would have ever chosen. Anyone have any idea what may have occurred?
