I can not get my game to recognize my side buttons on my mouse like it use to.
They would, i believe be Mouse Button 4 and Mouse Button 5, but I might be off by one number.
System:
Win 7 Ult. 64bit
4g HyperX Ram
Close to 2tb HDD
9500GTX Vid
Cable internet
Logitech MX518 mouse
Logitech G15 keyboard
Is there anything I forgot to mention?

You have two options that I can think of.

1- Do like I do, and use your mouse software (which should be included with it, or available for download) to remap the buttons to keyboard presses while playing CoH, and bind those to the actions you want.

2- bind a different test command, like "/bind button1 t $name, button1", to buttons 1-9, and press the buttons to see which is returned. There is a small possibility that your mouse doesn't use standard button mapping. If this is the case, this method will return nothing, and you'll have to go with option 1.

Oh, and if you try option 2, I don't recommend remapping button1 and button2. IIRC, those are the same as the left and right mouse buttons, and may mess up your game until the binds are reset.

Hopefully, you're not using Mouse Button 4 and such. It's simply button4, button5, etc.

Have you tried mapping them via the options menu? What do they show up as there?

Hmmmm.... I have a Logitech gaming mouse (forget exactly which one) and I haven't had any problems using the side buttons. So standard checks would be download the lastest drivers and install those and make sure that (as StarGeek said) you're using the correct button names.
