Next time, you should check right at the beginning of the TF. Any challenge setting is displayed with an icon under your hit point window.

Oh, I would very much want a "Master of Fail" badge. 
So, we just completed a MO 5th TF run, complete with no deaths, no temporary powers, only to discover that none of us received the badge. Head scratching ensues, until someone asks the team leader if he set the "Zero Deaths" challenge. "OMG!"
