I haven't gotten my 60-month badge, which should have awarded back on January 2. I put in a support ticket yesterday.

Thus, I have a feeling there might be a bit of an issue with B. You seem to have done pretty much the same stuff I have; namely, you added every single retail edition you could get your hands on in a very short time frame. I've had some issues with my last two Veteran Rewards (63 and 66 months) and had to send a petition for both before they were awarded, and I think adding so many retail versions might be what caused the snafu.
Submit a petition in-game letting them know (politely) that you should have been awarded the 3 month Vet badge on December 20. If you haven't gotten your badge or heard back from them by tomorrow morning, log into the support center on the PlayNC website and update your support ticket (rather than starting a new one) and let them know that you haven't received a reply yet.

I'm not sure on this, just hypothesizing. Either way, he'd know tomorrow.
OP: What does your "Vet Schedule" say you're due? If you look under "Game Details" on your NCSoft master page, you'll see a "Veteran Rewards" link, with info like this:

My Veteran Rewards box just has a link to the Veteran Rewards Page...
My total time says:
Total Paid Time: 0 Years, 0 Months
Veteran Rewards Time: 0 Years, 2 Months
Which doesn't make sense, because I payed for this month! It wasn't a free month like my other ones!
It even says on the billing summary page:
Jan 2, 2010 16806882 City of Heroes® [1 month subscription] $14.99 (USD) Paid
I'm sending a support ticket then...
If I could find the button to edit the one I sent in game on the website...

...Well, the CoV Box was due to the consolidation of CoV and CoH combined with a mass over printing, resulting in shops such as big lots buying the excess stock...
But still payed my 4 bucks! D:
I was supposed to have received my 9-monther on the 16th. I emailed ncsoft as wella s bugged it lol. Waiting on a reply....
The system is bugged. Log a petition and they will fix it for you.
Vet rewards seems to be having a issue recently. I was actually 2 months over due before I noticed and placed a petition. My vet reward was awarded the next day (on Christmas eve no less).
A petition should take care of it for you.
Mine got fixed a day or two after I sent in a ticket to support.
Trial time counts as well, since all you're doing is switching its status to full, not creating a new account. I started my trial Christmas Day, and that's generally when I get my year anniversary badges, though this last one was one I had to petition for.
Loose --> not tight.
Lose --> Did not win, misplace, cannot find, subtract.
One extra 'o' makes a big difference.

Well, I was granted my rewards (Yay!) and my problem is solved for now.
Until march, anyway.
Alright, so yesterday was my four month anniversary of playing COH.
I have yet to receive my 3 month reward...
My account history looks something like this:
