Okay, then did you check for either of these previously mentioned conditions: "Are you in an AE arc? TF? Ouro?"
These states do NOT reset when you log off. You can log in and still be 'locked into' an arc or task force. Even if you're alone (i.e. not on a team), you might need to open your team window and see if there is a big 'Quit' button.

As I said before, it happens even when I've just logged in. As far as I know I'm not involved in any story lines etc. Not sure what TF or Ouro are though.

Both of these, and AE mode (which is another alternate version of task force mode), play havoc with all kinds of things.

There is a bug in that the message at log in refers to the /respec command when it shouldn't.

Ahhh.. this is a different bug. You're actively involved in an AE mission (which is treated as a Task Force), or are in a Task Force. Since you don't know what Ouroboros is, you're probably not on a Flashback mission through the Ouroboros crystals.
In any case, YES do that; open up your Team window and see if there's a "Quit" button.

The Villain respec contact is Arbiter Lupin, who is steps away from Arbiter Frederick (the Trainer in the Nerva).
The wiki also lists Arbiter Sawyer, who is near the Pocket D entrance in PO, but I've never heard of him before. Is he new?
Yes, Arbiter Sawyer is new. I think they realized that sometimes even lower level Villains need to respec, and going to Nerva is a pain at low level.
Arbiter Sawyer isn't really new. He's been there for about a year now...lol

Anyone have any clue as to why I would get this message? Everytime I log in I get the message saying I have a free respec type /respec to start. So I decided to give it a go, get all my powers picked out, assign my slots and decide just to take the purchase price of my enhancements. Each time it says repec failed.

If it were a free respec, then '/respec' would work to start it. Since it doesn't, it's an earned respec and you'll have to go to one of the respec contacts listed above.

Unless I'm badly misunderstanding the OP's post, he's saying it DID let him start the respec, it DID let him pick the powers, it DID let him assign slots and it only failed when he tried to finish.

Anyway, I'll have to test out the login messages for toons with and without freespecs....

I had this same problem... using a free respec, I could go through the whole process, but in the end, "Respec failed". I petitioned tech support in-game, and here's some things they told me:
A (free) /respec can fail if you happen to have any unslotted enhancements in your enhancements tray. Try clearing out your tray, then /respec again.
It can also fail if you have levelled up, but not yet visited a trainer. So, train first, then try again.
Finally, moving to a different zone can often clear up whatever's interfering with the /respec. This solved the problem for me.
