"All Custom Characters" Faction
When did you first see this happening?
This isn't quite the same thing as I and others have seen since Friday's patch (see my thread here and Wrong_Number's thread here ), but it might be related.
There *was* a patch on Test during Open Beta that broke custom factions in the way you're describing, but I haven't seen the "All Custom Critters" bug since it was fixed on Test.
It seems it's been happening ever since I published when we launched. Would re-publishing clear it up? I haven't tried for a few days.

No idea. Re-publishing doesn't seem to have any effect on the bug that Wrong_Number and I have reported. Like I said, it might be something entirely different from what you've got going on.
Have you looked in your actual storyarc, critter, and group files on your hard drive to make sure everything looks alright in them?
Check all the critter and group files. You may need to delete the group and reform it, then republish the arc to get it to stick.
I've played through this arc twice, the first time (before the last patch) all the bosses belonged to <All Custom Characters>, and after the patch, at least one boss belonged to <Hippies>, but the EB still belonged to <All Custom Characters>.
Yes, I was one of the people pestering you with that piece of feedback, sorry about that, I didn't realize it was a bug and thought it might have been a mistake. On the bright side, nobody else on my team even noticed.

Clear out all your local MA files and then edit the mission and see if you can correct it that way. I had weird issues with an arc and was able to fix it that way.

I'll give this a try tonight. I know when I was checking out my DC arcs on test, I'd open them in the editor to verify that the right version made it up, and the powersets seemed right on critters, but they weren't when I played. I suspect the editor uses local versions of identical enemies rather than what's ACTUALLY up there, or some other craziness.

Try going into your My Creations and groups, then edit the group and make sure that all mobs are included in the group. Then go into your Arc, change the group and encounters to a standard group then Save & Exit. Go back in and change the group back your custom and choose each mob.
I had this problem because I was editing multiple characters for a group and forgot to change the group name on some of them as it defaults to All Custom Characters and not the group you are editing.
Okay, I tried clearing out all my folders (after backing them up, of course). Opened it in the editor. No problems there. I even picked some other character and back again to see if a poke would help. Republished.
I go in to play, and... "All Custom Characters" again. Jeez!

I'm having a lot of similar problems.
Some mobs end up as "All Custom Characters". In other missions, one mob will pick up the group name of a different custom critter group.
I've also had "standard" groups pick up the custom critter group name.
Quadruple-checked everything in the missions and everything is organized right there.
Tried re-doing groups and deleting and re-creating buggy groups. No joy. Something's borked. Fairly minor, yes, but borked.

I've been pegged for this enough times in my feedback that it's getting annoying, and I need a hand...
Arc is the one in my sig, the Psychotropic Hydroponic Freak Out. I have three bosses -- two are generic bosses from the Hippies faction, but given names. The third is also in the Hippies faction but is an EB.
Everything looks absolutely fine in the editor -- opening the goals in tab 2 shows them as being in their faction, not in All Custom Characters. But when people go to play the arc, they see them as All Custom Characters, not as Hippies.
What's going on here? I've republished several times, and I'm editing the one that's up there (not a local copy), everything SEEMS okay... but it's not.
