No you can get plus 1 from tier 3 destiny or lore as well. I believe only alpha shifts you in regular missions but see sig.
Edit: I looked it up t3 or 4 alpha gives 1 level shift. T3 or 4 dest or lore give incarnate shifts ie incarnate content only and they stack in incarnate content.
You get a +1 level shift by equipping a T3 or T4 Alpha Slot Ability. This is the ONLY way to get a permanent level shift in the game right now. You can also get a short duration +1 Level Shift by using an Ultimate inspiration.
Slotting a T3 or T3 Destiny or Lore Ability will grant a +1 Incarnate Shift. Slotting both will stack, so you'll have a +2 Incarnate Shift. Incarnate Shifts are like Level Shifts, but they only apply while participating in Incarnate content (Trials, Dark Astoria, the new Incarnate Arc coming in i24).
Incarnate Shifts stack with Level Shifts, so if you have T3 or T4 in Alpha, Destiny and Lore, you will be +3 in Incarnate content (but still only +1 everywhere else).

However, technically, there is no such thing as getting +2 and +3 with level shifts... as Incarnate Shifts are what you get from Lore and Destiny trees. The difference is that a level shift is permanent for all content (at 45 or above) while an incarnate shift only works on iTrials or in DA.
They apply if you slot the powers in question so you can put em on and take em off at your pleasure (outside of cooldowns). Also these aren't actual levels you are basically sidekicking yourself.
As stated currently the Alpha, Lore and Destiny slots all offer a +1 starting at tier 3.
The +1 from the alpha slot is active in all parts of the game level 45 and up.
The +1 from Lore and Destiny slots is only active in incarnate content that includes Dark Astoria.

It's funny. When incarnate slots began I was burning through them on a good number of my 50s, most of them reaching tier 3 or higher.
I've slowed down a bit but just recently got a tier 3 Judgement for a fairly newer 50. I was quite surprised that Judgement didn't apply a level shift. I assumed all slots did and capped at +3. Shows what happens when you don't pay attention.
So yesterday I got my tier 3 on Destiny. Finally a level shift for this character. I ran a regular paper mish, +4 difficulty, and had convinced myself that it was running much smoother than before the shift. And just now I'm learning that in non-incarnate content, only the Alpha applies the level shift. I just need to slow down!
I want to become level 50 plus 1. As we speak, I'm leveling up both musculature and Pryonic. Is the alpha path the only way to get my first plus 1? And if that's the case, what's the order of level shifting. From alpha to judgement etc?
