Graphic Issues - Good FPS but random freezing
Nothing's ringing a bell right off the bat, but I have experienced a similar issue on bad lag days. I did notice your memory related quandry. Well, I see it that way atleast.
I'm assuming you're running the x86/32bit version of Windows XP. You seem to have 3Gb of physical RAM and a GPU with 1Gb of VRAM. I'll try to describe what could be an issue.
From my understanding of Windows Memory allocations/equations, the OS starts with the GPU memory. In this case 1Gb. And then starts using up the onboard RAM. Depending on the system, you have a maximum of about 3.5Gb of total usable memory. With your situation this means that the first Gb of that is taken up by the GPU, leaving 2.5Gb for the OS and applications to share. While XP will likely take between 300Mb-756Mb, the game will try its best to take nearly 2Gb. If the OS is using more (I'm including everything but the game when talking about the OS using memory). Needless to say it's entirely possible that you may experience issues related to this.
P.S: look for HijackThis and CoH Helper in the forums and post that info here as well. the game doesn't use Direct X, but you are off to a better start than some.

As a side note i do use GameBooster to turn off alot of the extra junk when i play, however the freezing issue happens with or with out it on.
I contacted support about this and finally got it sorted out, did the msconfig and turned off everything except windows required processes and now i'm not having any trouble.

I would like to rule out the possibility that there is another program interfering with the game connection. Please disable all startup items to make sure that no background programs or processes are causing a conflict. To do this, please perform the following:
1. Press and hold the Windows key and then hit the R key. This should bring up the Run dialog box.
2. Type "MSCONFIG" without the quotes, and click "OK." This should open the System Configuration Utility.
3. Go to the "Startup" tab and click the "Disable All" button.
4. Next go to "Services" tab and check the "Hide All Microsoft Services" check box, then click the "Disable All" button.
5. Once these items are all disabled, click the "OK" button and you will get a prompt to restart your computer. Please restart your computer.

Also, be careful, simply wholesale turning off of services like that can do nasty things, like turning off your AV, 3rd party software firewalls, etc.
There are a few other settings that the posted CoHHelper indicates are at Very High that maybe should be taken down a notch if there are any additional framerate issues. Ambient Occlusion as well, maybe, should be dropped from High to Balanced. If there are problems, of course. If everything's been worked out then it can be left alone.
CoH framerates can vary wildly from zone to zone. I'll be rolling along with my at over 80 FPS with everything on Ultra because I'm in an older zone. Then I'll go to revamped Atlas and BAM! 25 to 30 FPS. And I've got a Radeon HD 7770 / 4GHz FX-6100 / 8GB RAM.
Yeah, there are a few settings there that should definitely be lowered. The screen resolution should be fine. I run a bit higher than that on a HD 4650 under Windows 7 64-bit. But I've also got over twice the RAM that you have.
I wouldn't worry about je-saist's info about the minimum being a 4770. I was told by a programmer that they were running on 4650's with almost everything cranked up when they gave us the details of Going Rogue at the second HeroCon. The only thing I drop the settings on are the ones I'll recommend below. Granted, I have more memory and a newer OS, but you should be OK.
I'd recommend dropping World Detail to High. Really, you don't need it at Very High. And some zones will cause more issues than others. For example, see ScottyB's notation about the revamped Atlas Park killing his framerate.
I have one recommendation for Ambient Occlusion. Turn Ambient Occlusion OFF. Completely OFF. No Advanced Occlusion settings enabled at all. That one is a big frame-rate killer.
You'll have problems with AO using the 4600 Series card. Especially with XP. I have the 4650 running under Windows 7 with more than twice the RAM you have and still need to leave AO off.
Water shouldn't be a problem at Ultra Setting. Shadows and reflections can be iffy at times so I've been running them at just High. I turn Reflections up to Ultra if I want to show off the reflective windows in buildings to someone by hovering in front of the windows of the AE buildings. I leave Bloom OFF because I just don't like it.
If the game spit out 20 dollar bills people would complain that they weren't sequentially numbered. If they were sequentially numbered people would complain that they weren't random enough.

The 4650 is not a bad little card, I got one as a replacement for another card that failed since it had a lifetime warranty.
Can it run UM? Not to a level where I consider acceptable. I have it paired to a C2D E6600 which is better than the AMD x2 3600 that the OP has.
I have to agree with what was said, settings are too high. Start with minimal graphics and start adding effects.
I find Grandville or the new AP or Imperial City to be a good place to test. If you don't get lag in those places, you probably won't in game during normal play.

CoH in WQHD
My settings are set to minimum and i still freeze up, going from 40 - 60 frames down to .5 for a good three seconds when ever i try to turn. It happens less often and for shorter amounts of time on the blue side but it seems like my card should be able to handle the graphics set to alot higher. Really when the card i replaced (geforce 9400) could do it just fine. I know i probably cant touch ultra but higher then minimum.
