i encountring this too and also can't pinpoint when it starts to go screwy 
.
the only thing i found that helps most of the time is before you alt tab press ctrl+alt+del to bring up the task manager box, 99% of the time it prevents this problem.
other than that i didn't find anything else that helps, hope this will help you out.
If you mean that say the screen goes black except for the UI/mouse, then you've either got the graphics too high for your system or perhaps (more likely) there's something else running in the background that's taking up the GPU's resources. I normally alt-tab fine ingame unless I forget to drop to reduced gfx when running two instances. Don't forget to add this to your properties on the launcher as it helps this a bit.

No, the screen isn't going black.
The screen simply displays a still image of the exact moment that I click back on the window, or alt-tab back, or return to CoH in any way. Another alt-tab returns functionality to normal.
When it does behave like this, it happens every time I alt-tab, with no exceptions. Normal -> Still image -> Normal -> still image, like clockwork.
When it doesn't do this, it never does it during a session.
I'm sure it means that there's something interacting with CoH that's causing this behavior, but damn if I know what, given that I'm always running the same programs all the time. (The only exception being when I have MIDS open for levelling purposes, but MIDS being open does not have any effect on this. Sometimes the issue is present when MIDS is running, sometimes it's not.)

This happened to multiple people quite a while back. I can not remember what the solution was. I do remember that cycling through alt-tab one more time was a work around.

What operating system are you on? My computer just started having the same problem recently, and I'm wondering whether it has anything to do with my upgrade to Win 7.
I've found that restarting my computer usually makes the problem go away for a while, which suggests to me that something running in the background is competing with CoH for resources. Danged if I can figure out what it is, though!
By the way, sometimes CoH freezes on the last image as the OP describes, and sometimes it goes black, so I don't think those things have different causes.

i usually run in windowed mode which gives me no problems switching windows
This is really weird, and I have had no luck in narrowing down what is causing this error.
I alt tab frequently when I'm playing, however over the past month or so, there have been times where I would Alt Tab, and upon coming back to the game, the display would be locked. I can still move my mouse, I can still hear ambient noise, I can still MOVE... but the display doesn't change. When I alt tab again and come back, everything is behaving as it should be.
Worse yet, it doesn't do this all the time, so I have no idea what's causing it. The only things that I'm running at the same time are Firefox 7, and AIM.
Has anyone else had this happen? Anyone know what's causing it?
