Am I having a GFX issue? If so, how can I fix it?

And you have both FSAA and Ambient Occlusion Quality set.
Try turning one of these two off.

Oh. There's your problem. Antique video card. Probably running newer drivers as well.
For that old of a card I generally stick with the 197.45 drivers that were the last WHQL drivers released before the 2xx.xx series of drivers.
Also, post the results of CoH Helper as described in Hyperstrike's [Guide] Asking for technical help on the forums thread. We'll be able to see your settings and likely be able to make better recommendations that what we do when we just guess at how things are.

Assuming I figure out how to install the old drivers on my system, are there going to be any new issues as a result of that?

Shouldn't be. Those drivers were released after the Ultra Mode additions, so those are supported.
I've had better performance and less issues on older video cards using those than on the newer drivers.
EDIT: Download the older drivers but don't install them. Download and install Driver Sweeper but don't run it yet. Uninstall the current drivers from Add/Remove Programs (XP) or Programs and Features (Vista? and 7). Reboot into Windows Safe Mode and cancel any driver installations that come up. Run Driver Sweeper and let it remove the remnants of the current driver installation. Reboot into normal Windows mode and cancel any driver installations that come up. Install the 197.45 drivers. I'd have posted a link to the drivers but I'm not sure what OS you have.

I have XP, but I'm not sure if I want to install anything on my computer until I've got some other issues with it resolved. You can go ahead and post those links, though.

Same exact problem, but I'm on a laptop with ATI graphics. (ATI Mobility Radeon HD 4200 Series.) This problem exists for me on the Medium Shader settings, but goes away if I bump it up to High. This does result in a significant FPS hit, but at least I'm not looking at the weird windows, lava trails and of course, the Dust Rifle's weird glow.
My driver version is 8.872.0.0, which I assume is up to date because the game doesn't inform me I have outdated drivers and the date listed is 7/7/2011.
Is a driver rollback something I should also look into? If so, what should I look at using? I don't necessarily mind running the High shader setting, but the FPS hit is *harsh* in First Ward.

For me, I am on a 64 bit operating system but do not have either FSAA or Ambient Occulision set.
Those drivers are not the most current drivers. Those are the 11.7 drivers. The 11.5 through 11.8 drivers had several problems with CoH. Those seem to be resolved in the new 11.9 drivers that were just posted on the ATI site. You can download the 11.9 Mobility Drivers from the ATI site using this link.
The "old driver detection" used by the game doesn't check to see if you have the most up to date drivers, it checks to see if they are older than a specified (and unknown to players) amount.

Just downloaded and installed the new drivers, but still no luck. Lowest and Low Shaders don't have the issue (because there isn't lighting on the windows,) Medium causes this but goes away as soon as I crank it up to High.
Guess I'll have to learn to deal with this, eh?
The night-time window problem is a known issue and will be fixed in a future patch.
