Working on a different build. Is the Panacea Chance for HP/End an always-on bonus as well? Or will I need to slot that into a toggle/passive?

Panacea is a proc so it's best placed in a toggle/passive.

Does this one stack with the steadfast protection +3% def all?
So in other words, using both, you get +6% defense to all?

Yeah, they do stack. But the Glad Armor unique is not only uberly expensive (2 bil on the market... can go up to 3 off-market usually), it's also very rare to find to begin with.

Duration mentioned? Proc/Proc120 (Examples : Miracle Unique, Stealth Uniques)
The word "Chance" mentioned? Proc (example : Trap of the Hunter : Chance for Lethal damage).
