I had that happen in my first Reichsman TF in like the 2nd or 3rd mission. I was so hoping not to die because they were trying to do a Master run.

Once I got the Mutation Booster, I must have used that power constantly all day trying to get the Rikti Monkey devolution, but no dice. So when I'm messing around in PvP, and click the power trying to get a bit more Defense... POOF. I'm in the middle of a PvP zone hoping around as a monkey trying to not die from NPCs or players.
It was quite amusing.
This sort of thing is why we need a Rikti Monkey Epic Archetype.

LOL Oh my I can imagine how that felt. So far I have been lucky .. shortly after I purchased that pack someone on the Virtue TF global channel related an incident that occurred while they were on a TF and facing the end game AV. They hit the mutation and devolved. Since then I have tried to be VERY careful to only use the power when I am in a completely safe environment. Of course this has forced me to either 1) not use it at all if I ran into an AV/EB and hadn't already activated it or.. 2) only activate it at the entrance of a mission before venturing inside.
By the way.. option TWO has backfired on me at least once. I entered a mission, hit secondary mutation, devolved into a Rikti monkey, and THEN the ambush that attacks you at the door arrived! Luckily I was on my MM and while my pets kept the ambush busy I was able to escape out the door to wait for the effects to wear off. :-D

If one starts Self Destruct, can one then cast Secondary Mutation? If so, turning into a Ritki Monkey and then blowing yourself up really ought to count for the Zookeeper badge.

The same thing happened to me, during a tip mission where I had to fight Ghost Widow. Just as she shows up, I hit Secondary Mutation for the buff and poof, rikti monkey. I had to run...er...hop away from her, hoping not to die. She followed me all over the map too.
