Try a Corruptor of some sort.
Just about any Dom Combo should also work.
Not sure if you meant not repeating the same combination of powerset or not using the powersets again at all. First thing i think with that name is demon/storm MM. It fits both the idea of creating chaos (summoning demon) as well as spreading chaos on the battlefield (storm). 
Or just make a SS/whatever brute/tank and name it K-O creator

Mind/Fire Dom wouldn't be bad. Mass Confusion helps spread a ton of chaos, and /Fire is just plain fun.

I have a plant/thorn dominator named Tel'a Jhin Va...but he is more affectionatly known by the moniker of Chaos Dragon. Seeds of Confusion at lvl 8...chaos. Carrion Creepers...more chaos. Water Spout from Mako's pool...omg make it STOP! He unleashes hell on the battle field. Not quite Stormy caliber...but he comes pretty darn close!

Hmm Storm is what I was thinking too.
I don't like illusion and I didn't really enjoy demons.
Bots/Storm I could see working although a Corruptor or Dominator sounds better as I don't have a good one.
My fire/storm Controller is a reliable generator of chaos. Thugs/storm or bots/storm would also be good choices.
Plant/Storm can make a lot of chaos.
For Corruptor Energy/Storm.

for now at least Chaos Creator the level 1 magic fire/storm corruptor praetorian has been created.
I'll probably delete him and make something else
Chaos Creator is my persona, my identity, my global, my forum name. it is me.
It is also my level 22 fire/fire brute which I'm going to be moving off my home server to make way for a new main character. The problem is I can't decide just what a Chaos Creator is.
I have:
SS/SD, SS/Elec, SS/FA Brutes
SD/SS Tank
Cold/Psi Defender
Fire/Rad, Fire/Kin Ill/Rad Controllers
Crabs
Bots/Traps, Demons/Traps, Necro/Dark, Thugs/Storm Masterminds
Claws/Regen, Spines/WP, Spines/fire, BS/SD Scrappers
Fire/Fire Blaster
I don't want to repeat any of those for Chaos Creator so with that in mind does anyone have some suggestions for complete eradication of my enemies while causing some chaos?
