Correct me if I'm wrong, but if you get your transfer intercepted, isn't it more a case of doing it wrong than a case of the interceptor knowing what to look for and doing it deliberately?

I've only twice made a transfer via the market (haven't really needed to do it, since every character can be self-sufficient), so I'm not sure.

Short answer: Yes.
Longer answer: If someone is actively trying to intercept transfers, they may catch some that weren't done COMPLETELY cluelessly, just 'kind of' cluelessly.

I've never had one intercepted. And I don't expect to be doing any for a while now. "Free servers transfers? Load up the influence mule and send him on a tour of the servers where I play!"

Having only done two transfers, I'm batting 100% on getting through without interception and probably only "partially clueless". (Compared to some of you, I may be "Very Clueless", but I digress. )
I avoid using high-level TO's, which are the medium I've heard suggested here, and instead focus on something else that's low demand. I find a vendor-trash lowbie recipe that has zero bidders and only a few for sale.
PoorGuy buys all of them for less than Vendor Price (let's say 500), puts one of them back up at way higher than normal. (15K?)
(Write down extact recipe and level on Post-it note)
Log onto Richguy and check last 5. If all of the Last 5 is 500 or less, buy one for a few million.
Log onto PoorGuy and check.

What am I doing wrong that could put me at risk?

Someone would have to buy your item then 'paint the tape', i.e. put in 5 more transactions that look right. One additional security step would be to open the 'more info' window and make note of the date/time of your last transaction and then check that after you log to the other character.

Another step I add, is I post three items, all at a very odd amount ( say 673). When I log back in, and there are still only three, I then try to buy them at one less than the amount I listed them at. If I cannot, then I try for one at the amount I listed. If I get one, then it's safe to transfer.

Will make note of that.

I would probably use a teen-level To-Hit DeBuff, Snipe, Taunt, Def DeBuff set recipe or similar. Something that prefereably has very few on the Market, zero bids and no movement for a long time.
And this would be at a time of day with low server population.

If I need to make a transfer I usually just use a training enh. Buy up any that are there. Then do transfer. Dual boxing or having a friend watch it for you helps safe guard it.

If you're dual boxing, couldn't you just transfer the inf from character to character?

Don't. Many people only play on one server.
Dual boxing does make a server transfer safer. It lets you have both the sending and receiving characters logged in simultaneously so you can WATCH the market continuously - there is no 'blind spot' while you switch toons.
