I made a Shield/Axe tank back in i11-12 then jumped servers n forgot about him until the recent free Server Transfer.
Hes level 35 atm and I had to respec him (he was my 2nd toon so made some huge mistakes).
I did the Winter Event - Baby mish with a 6-8 member team that included a Shield/WM Tank and a Dual-Blade/?? Brute and they were dropping like flies while I was in the middle knocking the mobs down and up. Was awesome
I took True Grit at level 4.
During the respec I took Tough at 32 and Weave at 35. Both only have the one slot (with End Reduction) but Weave will be getting another 2 at 36 and Tough another 1.
Im finding even with Stamina 3 slotted that im almost running out of END almost every fight at +0/x0 (only done a few mishs to test the new power selection - only used him for 1-2 hours so far). Im not even running Tough or Weave yet (dont need them on these mishs).
Im running Deflection and Battle Agility and AAO all the time (with 1 level 35 SO enh) and Active Defence is on auto-activate (3x Recharge SO so it is up before it expires). Also have Combat Jumping turned on.
Im looking at getting IO Sets into him but I only have 26Mil (again this was my 2nd toon so my WW skills were crap back then - lol).
Currently my Positional Defences are around 30% for each (exact number I forget) with BatAgi + Deflec + Phalanx + CJ running. My SO ENH are blue/yellows but as im looking for IO's I wont spend much money replacing them - do some radio mishs with Natural ENH dropping mobs
Id love to hear how you find Shield/Axe - maybe you could write a guide [I tried ages ago with a different AT and it sucked]
What are you hoping to do with the tank? More hp? More regen? More Recharge? Softcapped defenses? etc.
Throwing darts at the board to see if something sticks.....

Ok first of all I have to critique some of your IO slotting.
First thing that stung my eye: Mako's bite. And excellent set for a Shield character but for some reason you've left the 6 piece out and miss the best bonus Mako has to offer for you. Stick that Mako triple in your two attacks and that's 7.5% Ranged Def with two slots.
Second thing: full Scirocco sets? With this set, on the other hand, the 6th bonus doesn't do much for you. Psionic Def doesn't stack with Ranged def against Psy attacks and there really aren't that many situations where you meet purely Psionic attacks to warrant the wasted slots here.
True Grit: you could safely drop Rech/End or Rech from the DW set here. The 6th bonus is again pretty much useless.
Phalanx Fighting: the Defense of this power cannot be enhanced, so basically it's only good for a Kismet +Tohit or a LotG +Rech. Three wasted slots in this power. Which gets me to...
Build Up: stick those three slots from Phalanx Fighting here and finish your Gaussian's set to get 2.5% Def against all positions.
Where's your Steadfast 3% Def?! It is probably the single most useful IO for any Def based character that isn't already soft capped. Grab it and throw it in Tough, Deflection or True Grit.
After all that, I'll just note that I think Grant Cover is overslotted (LotG +Rech and maybe another piece of LotG for +regen is enough) and Health is way underslotted. I pretty much always 6 slot Health on any of my non squishy characters like this:
Numi: +Regen/+Recov, Numi: Heal/End, Numi: Heal,
Miracle: +Recov, Miracle: Heal
Regenerative Tissue: +Regen
That gives you some nice Regen to go with your Defense.

I see you did not take Swoop. That's certainly a legitimate decision, but I like the power as part of a hard-hitting ST attack chain for bosses and above.
It looks like this would be tough to do, but I would try to move Phalanx Fighting earlier in the build -- especially since you're taking Weave relatively early. It's passive, taking no end, and it's nice defense, especially on a Tanker. One slot is fine.
I'd also switch True Grit, which makes you tougher and costs no endurance, with Against All Odds, which costs end and only boosts the damage of your (at that point) two single-target attacks. You'll want AAO before Shield Charge and the AoEs, but it can wait until after Stamina.
Speaking of which, my build doesn't have Physical Perfection or a perf shifter +end proc and I do fine for endurance -- you might not need such concentration on recovery powers.

Both Battle Agility and True Grit can be--and should be, IMO--slotted for resistance. To get maximum benefit out of two slots, the easist thing to do is just add 2 level 50 resist IOs. (And I agree that True Grit does not need to be 6 slotted--4 slotted is fine.)
As DSorrow mentioned, you should definitely get the Steadfast +Def and 6-slotting with Gaussian's is also a very good idea. Once you've done that, you really need only one 6-slot set of Mako's, one 6-slot set of Oblit, and a 5-slot of Scirocco's to soft-cap to all positions. That gives you the option of slotting the other attacks with cheaper sets or for other bonuses.
You might also want to consider dropping the Oblit set altogether and instead using a 6-slot set of Touch of Death in one of your ST attacks; ToD has has the same defense bonus as Oblit. Oblit is a good set, but it is very low in end reduction, and I wouldn't put it in an attack you plan to use a lot. (And I certainly wouldn't slot more than one set.)

EDIT: forgot to mention that Against All Odds has the standard endurance cost for a shield toggle, and doesn't need a second end red slot. So you can use that slot elsewhere.

OK here is my current Tank build (note Im at work so the exact details of when I took the slots and powers may be a little off but should be fairly accurate - NOTE I took CJ much earlier but I cant remember when...).
My problem is END.
I was running in the RWZ last night and took on a Purple Boss + 2 orange LTs (random encounter).
I had no problem with Health as I rarely got hit, but after the 2 oranges where down (love AoO + BU + SC + WhirlingAxe + Sands Of Mu [have to be quick to get them all in though]) I was effectively waiting for my END to recover in order to attack the boss.
I am just starting to look at IO's (have 4 of Crushing Impact on Gash).
With CJ + Defl + BAgility + Phalanx + Weave my Def for all 3 Positions is around 36.4% with mainly yellow SO's.
Im planning on getting 2x ToD sets for an additional 7.5% Melee (taking me to ~43%) and 1 Mako for Ranged ~40.5%.
As for AoE im thinking of 2x Sirocco's for ~42% AoE (+20% Reg and +18% Acc) and only for 5 Slots (
Obviously I want to get my Def Softcapped for all 3 positions but I need Recovery.
Options???

Here's what I did:
Slotted Deflection: 23.41
Phalanx Fighting: 5 (slotting doesn't increase the defense for the player)
Slotted Weave: 7.83
Combat Jumping: 3.11 (slotted with one defense IO)
That's 39.35% from powers. Now add:
The Steadfast 3% unique IO: 3
You're now over 42%. Soooo close! And you haven't used any sets yet, just the one unique IO.
One TOD set for 3.75% melee (use one of the poorer attacks for it) soft-caps melee; add 2 slots to Super Jump's default and throw in a complete Blessing of the Zephyr set to cap ranged AND AoE.
You can't buy the -knb IO from Blessing for love or money; but you can get it with merits earned by leveling up in pick-up Task Forces. You may also have to get one or two pieces of TOD with merits. Boom, you're done -- frankenslot your attacks for maximum effect (and good end reduction!), not defense.
If the TOD is out of reach, slot your worst TWO attacks with Smashing Haymaker.

CJ I got much earlier than 35 I just cant remember when and forgot about it when I was rebuilding on MIDS so wacked it in there.

By the above I assume you mean 3-slot with Def Enhancments.
Im not sure if Im going to get a Travel Power as im using Ninja Run which is fine for general movement and Temp Travel Powers for the rest. As such Ill prob 4 slot CJ with 3x Zephyr + 1 DEF Enh. Save me a power choice (If I cant find a power I NEED maybe Taunt for a IO mule with 4-5x Perfect Zinger for 10% Reg, 5% Rech and [5 slot] 2.5% Dam???? - Ive already got 2 Recipes).
Also thinking of taking the Energy Pool.
Here's an alternative: BA has 3 PBAoE attacks, so slot 2 of them with full sets of Multi-Strike, which will take you to the soft-cap for both melee and AoE. Two slots of BotZ (minus the -KB) for Ranged in CJ and you're done!
That way, you'll have the merits that you would have used for the BotZ -KB, and you can use them for some other expensive IO you might like, one of the Recovery uniques or a LotG +Recharge, for instance.

Okay, but what's the enhancement percentages on the powers with Multi-Strike? Are you using 50s, or something lower for exemplaring? I like to frankenslot (or semi-frankenslot) the PBAoEs for the best damage and recharge, and good-to-very-good accuracy and endurance reduction. The area attacks affect a lot of foes and are big, flashy attacks; I try to get the most out of them. So I try to push my slotting-for-softcap sets into other powers if possible.

FYI, slotting a full set of level 50 Multi Strikes would give you around 74% accuracy, 49% recharge and 92% end reduction; with level 35s, 64% accuracy, 41% recharge and 81% end red. I'd think that would be adequate for most players, but if you're very focused on recharge it might less than ideal. I've never felt the least bit hindered by attacks with recharge slotted to those levels, but of course YMMV.
But if the OP doesn't like the idea of using Multi Strikes, there are lots of other options. A set of Touch of Death, 5 slots of Scirocco's and the two BotZ will work just as well, and again doesn't require spending a lot of infl or merits on the KB BotZ.
