My issues were no where near as extreme as yours though, but I use a GeForce video card. A low end 512 MB. There were several driver updates for mine this year and that fixed the small issues I was having.
When you get this glitch have you tried typing /unloadgfx to see if that fixes the problem? Also could this be a video settings issue? What card are you using, is the card really old, are your graphics way too high for your card, and do you have adequate cooling for your card/tower to keep your vid card from overheating? I know there's STILL problems with ATI cards with water and enhancements and god knows what else that's been there since launch, so that might also be part of it.

EDIT EDIT: If the issue is being caused by tabbing in and out of the game, then try playing in windowed mode to see if that solves your problem. If you're running Win 7 you can drag and drop the title bar of CoH to the top of your screen to get rid of the resizing arrows when you click an edge.

Yeah, I get it while alt-tabbing on my lappy. This is completely different though. New textures!
Doing /unloadgfx a couple times usually fixes the issue.
