What impact would that have on tells? So that means each character could have five names... for a potential of 180 names taken by a single person on a server per account?

You could always play guessing games with players too.Always wondering if they are an SG mate,or Friend with a different name.

And Kitsune Knight has a couple of good points which you didn't answer. That seems like a whole lot of names that will be taken up and there are already some people whining that all the "good" names are taken on the popular servers.
/Tells would be a potential issue - what if someone switches "costumes/identities" during a conversation - does the system just translate to send the /tell to the new name? or does the /tell just fail?

I am suggestion a costume name change whenever you switch costumes.I mean having one name when you are a scientist,and then having the same name when you transform into a beast is kind of bland and would make me loose interest in the whole lore of man,then beast.
