Understood, but would your targets getting buffed not be just as much of a challenge as you getting debuffed?

Not really, because a standard PVP tactic is going for the weakest link. Someone getting a crapload of buffs definitely is not the weakest link. I can easily just switch target every time I notice someone is getting buffs. That wouldn't make it more challenging for me, but I can see that it will make it easier on the people getting attacked, which is what we're trying to achieve.
I think maybe then, it would be best to do a mixed option with both buffs and debuffs that give for example a max buff of 25% and a max debuff of 25% to achieve your example of 50% buff(which is a bit too high IMO ). Tie that into a reward system that functions like this debuff = bigger reward, buff = smaller reward. That way you get the risk vs reward thing working both sides.

I've always been a big supporter of this. The only downside I see to this, is that people could exploit it to transfer cash or enhancements from one server to the next, which IMO is not a big deal, but I would expect the devs to have some problems with.
Doubtless this has been said before but the biggest obstacle to getting PvE players into PvP is that they don't stand a chance against Formula 1 PvP builds in the hands of skilled PvPers. I know there are plenty of PvPer who will help new PvPers with their build and tactics but the fact of it is that many PvEers don't want to change their build for something they'll only play for a change from their main game.
Perhaps a system that could analyse your build for PvP-centric powers, slotting, IOs etc and assign a con that reflects your build's PvP viability would help? There could be some sort of league system in play that rewards (and penalises) players depending on the league their kill belonged to.
I can't see on-death buffs helping the situtation simply because they'll be exploited from day one. Offering better rewards for PvP wouldn't be much of a draw for PvEers either as it'll be dedicated PvPers who'll be winning them more often than not.

On Accolades: Quite a few accolades only appeared in the game by the time a fair few of my characters were in the 40+ range. Villain accolades for example, only appeared when the 40+ content was added, so I'm sure I'm not alone here. I don't think it's fair that some people can use them when they are exemplared down, and others can't.
Characters can ONLY get accolades at a low level by teaming with higher level players that have the missions, or can help them farm the right mobs.
I therefore suggest ALL toons retain accolades when exemplared regardless of what level they attained it.
On set bonuses: I don't see them as too much of a problem, as you lose them if you exemplar down to a lower level. You want to play in a zone where PvP players don't have all there super set bonuses? Bloody Bay is probably the best one. It's always a compromise between choosing lower level sets to retain the bonus, or higher level sets to get the best % on your enhancements.
I hate 'dualling' PvP. This game is profoundly unbalanced against standard solo with different ATs. Mixed teams is where things start getting interesting though, and generally much more fun and a fairer time. A team of well-oiled PvE players wouldn't fair badly in the zones I feel. I imagine a few 'trashtalkers' would probably shout 'gank' though.

Seriously... I very, very, rarely take a non-lvl50 toon into the zones anymore, because I can always be sure of wracking up some debt. For something that is meant to be a fun distraction from standard co-op PvE, I find it irritating that I get punished for it. Shouldn't it at least be half debt like the RWZ?
On a different note I made a suggestion for 'Bantamweight Battle night' a while back (level 12-13 PvP night). I've had a very busy time of things in RL lately so haven't had time to organise this. I might get around to it soon though.
