Possible Bi-PB build....
I'm a Tri-Former myself so i'm not 100% on the viability of Hunam Only or Bi-Form builds, However, I think it would be a good idea to put a couple more slots in White Dwarf Antagonize if you plan on tanking for teams at all, i still find it hard work with mine 3 slotted! (though i have no idea where you'd get the slots from )
I'd also drop Quantum Flight for Hasten, i found QF to be a truelly awfull travel power & the end drain is just stupid! If you use it as a 'panic power' it'll normally drop before you're in the clear, Light Form makes for a mutch better 'panic power' imo.
Also from experiance, the human attacks often leave a whole in your attack chain, and as human form is your main damage form, this might be a worry (personnaly i switch to nova form in these instances), i'd wait on the advice of a fellow Bi-Former about that one though.
No Health in fitness? WHITE DWARF AT 24!!??? NOTHING ON TAUNT?!!!!???
Just a few probelms... also get rid of Quant flight. I'm a tri-former but to me most of that looks ok apart from the things i pointed out. I dont quite know why u have combat jumping though... if its a group that does tons of knock back i just go into dwarf. Also you dont need... i forgot... damn thats typical. Build up needs to be earlyer, its amazing.
Oh! I remember! Hasten. Dont get it, propably slotted PB attacks recharge in a instance. Take my Gleaming Bolt, for instance, praticly instance recharge; takes less than a second and its is just 1 SO recharge on ++ which is great. Anything else i think about i will put here as well, but this defiantly needs improving. In my Triform opion.

Combat jumping provides immob protection not knockback and with the IO he has slotted in it, it will have knockback protection too.
I'd definitely take hasten, probably at the expensive of Quantum Flight as has been said. I'd also use four heal/recharge IOs from different sets in Essence Boost and Reform Essence - give virtually the same effect as the way you have them slotted at the moment. I'd put those extra four slots into Dwarf. If you're solo and have to use dwarf, you'll take forever to kill anything with no damage slotted and you'll probably need an endred or two in the attacks to avoid running out of end whilst you are slowly picking away at them.

Ooops, didnt realise about combat jumping but never really used it me self. Dwarf doesnt really get immbilised anyway, at least it never has done with me at all. And me and the Leicester lad seem to have a bit of a disagrement about hasten as well. I aggre you need to slot your heal though.
It does take a while to kill suff with dwarf and it does run out of endurance quite fast so you have to be careful with that.

Hasten will mean that Buildup, Essence Boost, Lightform, Conserve Energy, Photon Seekers, Dawn Strike, Incandescent Strike and Solar Flare will all be available much more frequently for a start. It will also carry over in Dwarf, making that much faster at arresting things. Plus slots are usually hard to come by on a PB and Hasten will give the the equivalent of 2 recharge SOs in every power at the cost of one power and 2 slots.
It's not needed all the time but can be very handy.
I didnt bother with the human form shields on my Human/dwarf PB. If I can't handle incomming damage with the self heals I use dwarf or light form. It gets annoying turning all the toggles back on after being mezzed, also not having human shields probably saves some endurance for atacks. I really like pulsar, seekers and solar flare as panic buttons too.
My Badge is human/dwarf and ive gone with Hasten over QF. In some arcs when i had to solo EBs i found it essential to boost my Dps since I had to finish off the EB before LF dropped, and reds only helped so much, and not always available.
I did try going with CJ and acrobatics for a while but realised that to many mobs stunned and dropped my toggles, after that I decided to handle all mobs with mez in Light form and Dwarf.. Because going with jumping pool really dont help anyway.

How does the above build look, going for a mainly human form....with dwarf as back up for teams and high mez mobs.
Mainly want to team with toon to get the most out of the inherent ability...but also for use solo, for when teams are in short supply (basicly, I want it all )
I know that the shields don't give a lot of resistance, but every little helps...would like to have gotten Hasten in there, but for the life of me couldn't see a power to drop.
What are the general thoughts?
