NPCs in walls, ceilings, floors,shelves, etc

I have played COH for 4 ytears and in all that time I have had to contend with NPCs stuck in objects or getting knocked into objects, where they can hit me but I can not touch them. I and other people have repeatedly reported this and still the problem exists. Doe it matter to the game managers that flaws exist or what is really going on that such errors still exist after all these years?

This error is present in a great many games, and is very difficult to fix. Standard code rant applies.

The issue is probably low on their to-do list, as it rarely happens and is easily fixed with a quick petition.

Besides a petition, the foe will usually come out on their own if you leave them alone for a minute and break LoS. Go to the next room or area and come back for them after.

That's because it's easier to add a "move critter back on to map" function that runs periodically on the server side of things than come up with a fix that doesn't adversely affect performance to prevent critters from being pushed through the microscopic cracks in the geometry (actually floating point math quirks) in the first place.

Other than the above fixes, you can usually drop/use some kind of PBAoE power and it will hit. This sometimes causes the critter to come out of the geometry.

Oh.. and if you have pets, you can summon them right in the corner of the wall and they will summon "outside" and attack the foe. (in caves)

It is not easily fixed with a quick petition. I report it when it happens and have only had help maybe two times but the problem is frequent.

I use the LOS ploy but it does not even work half the time.

This. I've had a computer opponent get itself stuck in geometry in UT 2004, as well. Not to mention having seen wrestlers clip through each other in certain situations on Smackdown vs. Raw 2009. Clipping errors are not going to be completely gone from games.... well, possibly not ever.
