Allies at Mission Start?
To get them to Follow you, you actually have to go Rescue them by standing next to them. I guess we are rescuing them from the boredom they endured waiting for us to finally get zoned in.
Also, there is often no way to force the Ally to be the first thing you encounter in the mission.
We just don't have the tools (Yet) for that fine of a control. It's something we may get in the future if they can figure out how to make it available to us, but there are no guarantees.
Also, make sure you are using the Ally section in the Advanced area and not the Rescue section. They will behave differently. I'm not even sure if a Rescue can be set to Single since I've never looked at those settings.
If the game spit out 20 dollar bills people would complain that they weren't sequentially numbered. If they were sequentially numbered people would complain that they weren't random enough.

Yeah, I use Ally settings, Rescue's wonky. Well darn, was hoping to have my ally spawn near/next or automatically run to the player. Although that could be bad if they run through mobs to get to you >_<

Some outdoor maps (like Steel) the front feature is bugged. If you place ally in Front they will appear in Back and if you place them in Back they will appear in front.
Follow does exactly that. They aren't fighting because you have them set to Non-Combat. Set to Defensive. Aggressive is crazy frothing at the mouth aggressive that everyone hates so much. Haven't tried Passive but that might work for you too.
But since they use spawn points they won't ever spawn close enough to the entrance to see you as soon as you enter the mission. Plus some players don't like/want allies so giving them the option of taking them is a good idea IMO.
Powerful allies steal experience/tickets. Many powerful allies and it is like not playing at all unless all you want is badges. Best I've been able to do with making allies not overpowering is to give them a healing/Buff set with a Control primary. They will still do way too much damage (IMO) but they won't kill 2 out of 3 mobs per spawn anymore.

In one of the cutscenes, the game hints that it ignores ally placement for outdoor maps, regardless if you choose Front or Back.
I've tried to do this with the current mission I'm working on, but for the most part I just put them close to the entrance, and avoid outdoor maps. Besides, I usually just surround them with enemies like they went in first and got cornered, and that's more of a challenge anyway.

This may be true, but it's also a lot of fun. A huge group of EB level allies beating up on everything that comes along can be mass chaos. And if you're just running a local mission, not publishing it, you don't get XP anyway, so you might as well do whatever is fun.
I'm being careful about my allies in my "real" mission, though. There is an ally for most of the arc, and she's Boss level, but whenever other allies are added, I am making them weaker, either Minion or Lt level. This puts more work on you as it takes effort to keep the allies alive, even if they're just on Defensive and backing you up. (Plus, your main ally is a really weak Controller like you suggested. Since she's portrayed as new to heroing and unsure of herself it fits the story)
For the final battle, I will have the allies show up right when they are needed, as an ambush on the boss. That should keep it from being too much for the rest of the mission, but give a fighting chance against an AV. Or at least, so I hope, I haven't tested it out yet.
Is there a way to get an ally to 'see' you at mission start and either come to you or spawn next to/near you? I would like to use a few outdoor missions, but when I put my ally on Single Follow, they say their lines, but just...sit there...waiting for you to come over to them.
Would an ally ambush work? How would I go about setting that up correctly? Thank you in advance.
