I will compare your build with mine when I get home but from what I see so far in your build.
Your positional defenses are strange. Ranged is way over while AoE is a little short of soft cap (%45) a good number to aim is %46 (45.5 will do too if defense debuff prot is %95) for each since you have a good (over %90) defense debuff. for survival depend on defense and resistance first regen last, getting hit less for less damage is always better getting hit less for more dmage but healing it out.
Not having perma-hasten (having 5 second difference acceptable 30+ is not) means you can't make your top chain GD-GC-SD-GC (order can be changed to GC-GD-GC-SD but its point is same) all times and this will drop your DPS in general. you can get LotG global recharge ench from a.merits with tips and exchanging reward merits to a merits you can get 5 of them in a week for a price of one recipe or you can get all five in 10 days by just doing tips.
You don't need kismet you can easily keep same amount of bonus with focused accuracy or tactics.
Having both SS and SJ unless being a concept is a waste of both slots and power keep whichever you want and get assault, tactics or focussed accuracy instead.
Your damage is too low (both burst damage and DPS) build up only kicks in every 30+ second and proc in it only once in every 20 (600 seconds or 10 min) for 10 sec (5 sec for proc) so leaving it out of equation you only have %8 global bonus to your powers.
I assume there is a budget issue otherwise don't franken slot purples less than 5 of them together. Usual purple sloting are GC having hecatomb with achielles' heel proc and GD having armageddon with Gladiators proc(though you can skip GD's proc if you have a budget issue and put slot somewhere more useful)
There are many slots with only one set enchantment in them with one type. You can easily change them to I/O's or something from a double or triple bonus ones. For example you can put two I/O defense rather than two sets defense enchantments and it will even increase your defense bonus since serendipity is a low level set.
I think there are more these are just the ones that caught my eyes. -
Time to clean my 50 ninja/ff from all those dusts. I think she was at 600+ days give or take 100.

Ok this is last one and I am quiet pleased with numbers on this one. I thanks Dech's for posting that last build as well as providing suggestions and anyone else who tried to help. I am posting this as final one I know there are some places that can be improved but I want to try this for a few week and see how it will goes.

Except max health (16 hp or so and having better regen makes it worth), eclipse recharge which is still perma, dark nova detonations recharge (around 0.40 sec gap between dechs and mine although it looks close to 1 second i am playing a way slower one at the moment and even now I don't need to use detonation more than once on same mob thanks to pets and mires) and stygian circles recharge (around 0.50 second gap which is not that big of a difference either), my build has better numbers.
Future plans for this build is to get another purple set probably absolute amazement and build around it but I don't think i will try it for at least a few months. -

I only have Health & Stamina Single-slotted, I think. That is sufficient? My Human Form is only a buffer, pretty much.
Solo, I spend most of my time in Dwarf or Human Form and on teams I switch between them all, as a true MFing Warshade should! -
Ok here is what i got after suggestions. At the moment if I am not mistaken even without any recharge ench in it, eclipse is perma (with hasten and incarnate) but i am not sure about it if its true though i prefer a suggestion on a set that i can put on it.

I do not believe you can come up with a tri-form build that softcaps one type or position and still has permanent eclipse.
I would love to be proven wrong.
One thing that bothers me while playing with WS is long form change animation. Its not a big deal while soloing but when with a team things die too fast before i can even change into dwarf or nova. -

Ok this is what I came up with. I am really suck at these things, I guess since I end up with over-excessive recharge with spiritual alpha. I know, I said 3 purples but since I saw that I can get 4 pets for almost 20 second (someone need tocheck my math but I am positive that i can get 4 pets at least for 10 second) i think as well put that purple on dark extraction.
Code:I really wish I could get some slots on quasar and orbiting death but frankly I don't know what to remove for them. I guess it is a little cost to get 4 pets but I am not sure if it worths that way. See thats why I like you, Dechs. You removed almost all question marks in my head with a few sweep moves.
I mostly play nova/dwarf but sometimes just for fun i get on human drop orbiting death and some other toggles and see things die. Oh i use double mire quasar on occasional base too but its a lot less than when I first got quasar (my build mostly lack even SO's at the moment but I gathered a good amount of inf just to build it from scratch. I realise today in a lambda that dwarf mire can be stacked if there is enough recharge. Together with with reactive proc, i may aim on that end as well. -
Well its pretty self-explanatory. I read dech's guide but I always prefer personal opinions rather than a guide.
What should i be aiming with a tri-form build? I already know having bazillion recharge to keep hasten and eclipse perma is a must but what else?
Positional soft-cap or situational (was this correct name for S/L fire/cold etc ?) soft-cap for defense.
Should I focus on damage or try to get accuracy and let mire's handle rest?
What should be focus on for human/ nova/ dwarf forms?
My toon is almost completly IO free at the moment with at least 3 respec I have a good amount of inf and intend to get at least 3 purple set I can get 5 purple set too but it may give me a little hard time to get pvp recipes if i do so. Also I don't know which will be more effective, distributing purples among forms or keeping all of them in human?
Which incarnates would be useful on tier 3 and tier 4?
Any other advice you can give to me is very appreciated.
EDIT: Oh and forgot to ask but what are top chains for each from and how much global recharge needed for them? -
Ok lets see about some definations first.
Random: things occur without order or apparent reason.
pseudo-random: describes a sequence of numbers which has all the properties of a random sequence following some probability distribution (except true randomness), but which is actually generated using a repeatable deterministic algorithm.
Computer programs uses pseudo-random to create a randomness image. Humans can't write a program that works on a total random since you can't describe it to a computer to compute.
So how does it works, there are a few ways the one I know is; producing a long enough string of numbers (most of the time diving numbers equally across in the string and keep repeating them like 10 times so a string of 1-10 should have at least a string size of 100) than multiplying the next number in the string with systems clock in seconds or miliseconds and diving number to a big number to get a number between 0 and 1. Than check number with a table for example if its between 0 and 0.45 than give common, 0.45 to 0.7 uncommon 0.7 to 0.9 gives rare 0.9 to 1 gives v.rare.
Taking above example people may get same type of rewards over and over because they are a little unlucky hitting at the same portion of string and system time. Considering there are more than 2000 player in CoH (don't know exact number) and assuming that each have at least 2 or 3 level 50's we will have a pool 6000 if string was not long (lets say a string of 500 would be too low for this) enough or somehow repeats similiar behaviour during its lenght (for example if string has a few numbers consecutively 0.45-0.7 results when multiplied with system time) than it may look like it has a pattern which it doesn't and it will be hard to find where is problem.
I am not saying CoH's random table works like this but its most probably a similiar algorithm which results in such behaviour and people are only seeing this behaviour, this doesn't mean system working wrong or right its not easy to tell this and if its really not dependant on participation as a metric than its almost impossible to see where is the problem (for example the ones I mentioned above) unless you have hundred-thousands of data if not milions. -
I think it was an answer for OP's some tanks can't even tank marauder statement.
Although its true that you can tank with a S/L set, it wont be effcient and any other brute or tank may just steal aggroo of him from S/L tanker. -
One thing is I dont plan to use DA asit stands out i already have PB, hasten, conserver power, build-up, barrier and god knows what i forgot to disrupt my main attack chain and i really don't need DA to get soft-cap in SR.
Another thng is build-up proc in build-up something i tried and it drops over all dps badly unless build-up can recharge under 10 seconds.
For endurance its really a hardcase most of the time i dont need anything and i rarely have a need for a blue inspiration. Only time i have an end issue is while doing an S/L farm in AE and its nothing that a blue insp can't solve. -

Removal of Assault and Tactics. Tactics wasn't even slotted, no sense in it's presence in the build. There's already other sources of +Perception, and a Kistmet dropped in Combat Jumping gives you similar +To Hit. Swapped into Spiritual T4 which makes slotting choices a bit easier, gives you true Perma-Hasten, and dug around for +HP/Regen. 49.4/47.5/46.7 MRA, add another 15-30% for the addition of Divine Avalanche in the build in situations that'd warrant it. Although this is ... only really big for content below 50. 50+, Barrier will easily provide a consistent source of +Def/+Res, as would a T4 Clarion w/ Secondary Bonus that could pop you to ~70% while active. This build does come with the change to Alpha, too. Switched from Musculature to Spiritual, but this is with the intent on Survivability being the issue. HP and Regen are increased, balances end use a bit, and with Rebirth, you'd be looking ...being practically untouchable if you choose that route. In sync with a T4 Reactive and the two -Res Procs in the build, the lack of Musculature could be irrelevant to overall long-term survival. The largest thing the build does is balance slotting across the build. It would be possible to manipulate afew areas, take out DA, and return a few 5% Rech Bonuses if you were to try and get back Musculature. It's possible, this was just the faster answer. -

I maybe just unlucky for a few days with %45 but when I bring my builds def to %47 again things turned to normal so its better to be on the safe side if i can afford it.1) Only a few enemy groups have to-hit buffs. Devouring Earth and Praetorian Resistance are all I can think of right now. If you think enemies have small to-hit buffs because they have up to a 10% chance to hit you in your combat log, it is really because of accuracy and level modifiers.

After tweeking alot in mids I finally manage to get some things sorted out but since I don't have anything to compare I wanted to ask people in here.
What was the goals of build;
1) Softcapped positional defense. I can't get enough satisfaction with this yet mostly because many mobs in the game have some sort of to-hit buff if possible I want to increase melee and ranged over %47.
2) perma-hasten. I wish I could do this without spending slots in boxing since I don't intend to use it and putting a purple set in it just to get recharge kinda seem sad.
3) Good hp and regen. If possible without using perma-hasten I want to increase these two so over all survival increase for this toon.
4) S/L res. I am not sure if I can bring this up without sacrificng anything else. I know active ones in SR gives a managable res when health is low but its unreliable and often a way to provide a small window to use inspirations.
5) Increasing overall damage. Since I get hasten-perma with this build I don't think I need anymore recharge bonus unless its possible to make top chain from GD-GC-SD-GC to GD-GC-SD. I can manage last one even without perma-hasten (my current build without boxing sloted with purple has %121.1 s recharge time on hasten) if I got speed boost from a teammate but than again its not a consistant solution. I had %34 damage bonus without build-ups before putting purple into boxing.
6) A suggestion on incarnate slots would be also nice I currently have %45 muscular alpha(tier4), %75 DoT reactive (tier3), perma-barrier(tier4), tier3 clarion, tier 4 ion.

and

The first only sets me to follow and the second only makes the pet go. What am i doing wrong? -
as someone who have a kat/SR main I think your build is quiet different than mine although almost same power choices. I got assault instead of elude and that's all.
A few thing that caught my eyes in your build and in your statement;
High Accuracy: IMHO you don't need this as much as you spent IO's and frankenslot. Katana already drops defense with each attack and with its high recharge rate.
Add damage: You are saying to add damage but you didn't even put hecatomb or aramageddon damage in your powers.
softcap positional defence: Although its true that %45 is soft cap its better to have something between %47-48 too many things in the game have high tohit buff's and i realised thet when I run with %45.x positional defense I got hit alot more than when I do with %47-48.
My suggestions may take a long time instead I will put my build and let you decide where to go from there. I can run gc-gd-gc-sd attack chain without a problem its not perma-hasten but pretty close currently working on it to make perma. It has better damage and slightly better max health, regen and resist.

Code:EDIT: one more thing if you are slotting boxing solely for set bonus than do it with purple it will give you more room for removing other recharge sets and replace them with damage sets. I know you said only two purple set but absolute amazements most recipes are cheap compared to armageddon or hecatomb. Just for that comment I am really tempted to play it :P
I made a demon/thermal at the moment damn RL doesn't let me get on much to make and try others yet. With shields it feels like i am playing FF again but its better this time with early healing powers and i need to get used to my pets are not melee anymore otherwise I enjoyed it so far. I will try to make and some of the other suggestions in here. -

Ok second joke aside I think I will try demon/thermal, demon/storm and necro/storm.
Just out of curiosity how demon/pain would be i think conceptwise it fits but I am not sure how it will be played. There aren't much demon mm in my server (actually not much mm at all but lets notdelve into there :P ) -
After a long (last was when SoA came out just so i can get SoA) time I decided to try my hands on villian AT's again. MM was my favorite probably still favorite but being that said I didn't tried much of it. My only 50 in villian is a ninja/FF (don't throw stones along with swords it hurts)
So I want to know what would be an interesting combination and don't say bots/trap bots/FF or any thugs/ i don't like playing things that I see everyday around. -

Even then I prefer to a) focus on S/L first and b) not rely on DA or Parry to land to get defense. Not becuase it's not viable, it is, I just prefer to go in and not worry about clicking X to maintain Y. I just want it to work all the time.
Funny that I still like SD and SR despite this...
