As far as the "death badges" they are for debt repayment, they were added in Issue 5. The text on them is misleading as it says something about dying so many times, but they are for repaying your debt.
An added note for Badge hunters, Operative Rutger in Nerva offers a mission to steal some jammers from Crey, on a 3 person team set to Ruthless (equivalent to Unyeilding) we saw Paragon Protectors in most of the mobs, I am sure that an 8 man team on Relentless could farm it for their Infiltrator Badge if they were so inclined. -

The story arcs that give you a badge. In order to get the badge does the entire team have to be there for every mission in the story arc or just the one that the get the badge at the end of?

I'm asuming it's the same as it is in CoH, and you (or the rest of the team) only need to be present for the mission that actually gives the badge to receive it.

The team only needs to be present for the badge mission itself.
On CoH I was able to get Negotiator on two fo my toons at once, I'm not sure if it was a bug that has been fixed, or not, but it was possible. Gather a team, enter the mission, log out without quiting the team with the mission holder ( important : you MUST enter the mission with the mission holder and have the team inside before logging out). Log back in with an alternate toon that you want the badge on, and get re-invited to the team. Complete the mission, and both the toon who completed the mission and the original mission holder should be awarded the badge.
This worked about 3 months ago on CoH, I haven't tried it since then, but I'm betting that it will still work for the moment. Its a good way to get badges that you missed with one character, and want on multiples. -

Villains can't do PWNZ badge as of right now, so the Chief Swiper should probably be taken off the CoV list, but it isn't that important to worry about. -

That would be nice. I don't think it will happen, but it would be nice. My problem is that they keep running off on me, and they don't come back that often. I keep using taunt, but they just wander off and disappear. -
Mongoose, the Snake Kill badge, is for 25 Snakes, which you easily get during Kalinda's missions. The Snake Gladiator badge is linked to the Snake Charmer exploration Badge, not the Mongoose Defeat badge, so with some running around you can get that one without a problem.

I have the first mentoring badge and half of my progress bar on my second, at least one other person that I team with on a regular basis has the second one as well.

I plan on doing the same thing. I'm starting up my dominator so my Brute can get all of his badges. Once you are out of CaD it is a bit easier to catch all of the missions in the 20's, but you can still outlevel them if you aren't careful. I missed Slag Reaper and Agent of Discord, but have the others qued up or completed for the 20's/early 30's.
BTW, is there a Badge Channel for villains on Freedom, or is everyone planning on using Freedom Badge? -
I was able to do Archmage Tarixus' missions at 25 with only the lorekeeper badge and I do not have the Weed Whacker badge yet.
Perhaps you have found some sort of strange bug. -
I have gone by the Succubus Respec contact myself at level 28 and she told me to come back at 30. So, for some reason, she is giving the option of the respec trial at 30+, but you cannot claim the respec reward unless you complete it at 34+.

I am horrible at navigating the zones via coordinates, can someone give me a hint as to what Crimson Revenant and Arbitter Leary are close to? Are they on Building tops, or on Rocks, which island, etc.?

An additional note:
Energy Cloak in the Energy Aura set gives Defense to ALL. So Toxic and Psionic damage are covered in the set, just not until 20th level. -

only to find out the only thing i could do was get the Fort Knox badge because there is a glowie stuck in a wall somewhere and the mission is not able to be completed

Same thing happened to my wife yesterday, she ran it twice with a stalker to assist her, and can't get a mish complete for it. So it is still bugged apparently.
I did the Virgil SF in CAD yesterday, and it is now giving the Binder of Beasts badge. -
I did the Virgil SF twice yesterday. I was 21 and the leader was 22. Anyone over 20 is auto-malefactored to being 20 and gains infamy or pays debt.
You do get a really cool Binder of Beasts badge and get an SO +3 to your current level. -

Same thing happened to me. I clicked on the door and got booted out with a Mission Failed message. -
I was on SC 14 with a pair of friends on my team. I was a Dominator, who repeatedly was dominated by an electric Blaster. The Heroes in SC14 were an organized cohesive engine of destruction, where the villains couldn't get a good organized defense together until late in the game, when it was too late.
Lag was a major drawback, and really turned me off to the idea. But reflecting back on it a couple of days later i gotta say i think it will be much better on the live servers when the load won't be so heavy. -
I am all for this change, My SR scrapper only runs the toggles, so I won't be reaching the DeBuff resist cap, but I will enjoy the resistance i do get. I'm in my mid 30's now and am now experiencing more and more earth thorn casters in my CoT missions, let me say that stacking Quicksands have led to my demise too many times for me to count.
I understand the controllers' complaints about this change, because unlike Defenders their Debuffs are not unresistable. My suggestion would be to make a small portion of controller debuffs unresistable, that way the SR scrappers and Ice tanks can still be protected from most of the debuff, but the Controllers don't feel as if they have been neutered either. it would keep both camps at least marginally happy. -
As much as i would love to see pistol scrappers too, i'm betting it is one of those sets that has been up there for a long time that will never get done. Statesman has admitted to liking the idea too, which may give us some hope, but not much. Personally, i'd love to run around AP pistol whippin' Hellions with a Pistol/SR scrapper.
