While nuclear weapons are indeed viable threats, even in a world of super heroes, I never could help that the OP is right, and I feel this is one of the major failings of City of Heroes.
We live in a world of super powers, but everything is so damn normal. Why aren't the Steel Canyon Star Scrapers with things like Buddhist temples or an arbitrary rural house? Where are the buildings that have garden terraces and grass top roofs with trees growing out of them? Why is the military so woefully plan in it's technological capabilities compared to the guy who's been building a suit of powered armor in a cave with a box of scraps? Why aren't the tanks equipped with high-powered railguns, capable of hovering off the ground (with jumping abilities), and armed with anti-aircraft missiles and miniature versions of the GAU-8 Avenger Gatling gun? Why do we have these dumb rotor-based helicopters and not more jet engine VTOLs like the Sky Skiff or Longbow Chaser?
I suppose, as pointed out, it's to try to keep players from disconnecting too much with the reality the game is set in. If things were too super, it would seem less like a super powered earth with everyday magic and super-science and more like some alien world with poorly planned thematics. -

As a random note: prior to the Minds of Mayhem trial released, I had this idea in mind that it would have been really cool if before Prate Penny got mind munched by Tilman, Metronome managed to psychically reach out across worlds to his primal self and the self that was the sole ruler of his own dimension.
all three would have pulled their minds into one and suddenly become an unstoppable new threat between Praetoria and the Battalion as the guy would just utterly tear apart a lot of Praetoria in a twisted attempt to protect both Penny's.
in my head it was sort of like the Super Seveteen Saga from Dragon Ball GT. -

In addition to some of the others mentioned, I would like to nominate a revived Clockwork King.
Simple enough for him to get his body back, we could even make it an SSA: Crey needs help stepping of their Reverent Hero project and decide the Clockwork King would be easy pickings, first they start by working with Malta and player villains to try to drive a wedge between him and Penelope Yin, heroes have to try to keep the two on more stable grounds, but the wedge still manages to become just enough of a divide that they both blow eachother off for awhile.
Crey swoops in, takes the clockwork king and lock him up and prep him for the Reverent Hero Project. Malta backstabs Crey to try to take the clone, villains take advantage of the confusion to have a technological shopping spree (loot dead malta sappers: get sap stick temp power!) while heroes are just trying to find out what happened to the King.
They find his dying 'body' next to his cloned one, Penny appears, distraught and guilty, but tells you she can transfer his consciousness into the clone, but as she's doing that, ambushes spawn, heroes trash the mooks, fight a boss, the Clockwork King is back, with a new body, and a much more stable mind.
As a gift and since villains would get a King's Standard buttload of temp powers from this, Clockwork King and Penny even things by rewarding heroes with a few temp powers.
Roll credits, we now have a new Phalanx member, and Penny and CK's relationship is now...
Freakier actually, because if my estimates were right, the clockwork king died when he was 14, sixteen at the most and Penny is about 22 now. -

If the Freedom Phalanx were as unfailingly competent as Sam expects them to be, our presence would have been completely pointless in every one of the arcs. If they did all the things Sam suggested, what do WE do in the meantime, other than stand back and watch them win?
The point is WE are the heroes of the story, and in order for us to be the heroes, the Phalanx has to screw up. If they didn't, the entire series of story arcs would have been nothing more than us tagging along while the Phalanx handles the situation.....again.
As Redlynne sarcastically points out, how fun would that story arc have been if our entire involvement in it was to stick Malaise with a tranquilizer?
Or in number five, we exit the cave and either that whole scene had been playing out while we were fighting our way though (in which case a "meanwhile" cutscene caption is needed) or our characters are just standing there, watching this with no real explanation for our lack of action.

He walked into it, probably fully aware that Wade had set a trap. Why should he think that Wade can do anything to actually hurt him, let alone kill him?
As I mentioned before, Wade's plan worked because he did a decade's worth of research into the psych profiles of the Phalanx members. He KNEW how they would react to everything he did, and he played them like a fiddle. He probably DID have a back up plan too, except he didn't need it because Plan A worked how it was supposed to. The player character is the wild card, the thing he DIDN'T plan on or come up with a contingency for.
Wade did everything right. He did his homework and took his time setting everything up just right. He also camouflaged himself by posing as a powerless relic thief. Recluse would have failed if he'd tried this because it would have been expected from him, Wade pulled it off because everyone underestimated him. Including us. How many people posted saying "I can't believe it's Darrin Wade behind this! He's a nobody!" If I were planning something like this, that's what I'd want everyone to think too.
Like I said, in that scenario, EVERYONE looks good. And in it's own way, I think it would be more impressive of Wade to have a genuine backup plan unlike a lot of typical villains rather than rely solely on one plot, and to effortlessly shift to plan B when plan A fails instead of him racking up a chain of flawless victories, because it shows he really did plan this out, and why he's a threat.
And if the players become the ones to keep forcing him into plan B, or in the case of villains, help plan A go off without a hitch (or even be forced to improvise and help secure a plan c) it makes the player feel like they are affecting events than just observing. -

Basically, I don't think they're incompetent because of bad writing, I think they're incompetent by design.
I could be wrong though, but my storyteller's gut says otherwise.
For example, with the fifth part of the SSA, I had proposed this scenario for Statesman's death:
Players exit the cave, cut scene begins as normal, Darrin Wade then tells States he does kill... and nothing happens. Statesman than hold up a glowing chunk and taunts Wade that he figured out this was some kind of trap, checked around before hand, and smashed it. Wade is forced to 'retreat' with the cover of a small army of Rularuu that you aid States in taking down. Then, Wade reveals he had a backup plan the whole time, just in case his first failed, and now he caught you too! States dies, but before Wade can get to you, you break free, and prove yourself to be a lot stronger than wade had anticipated. then it proceeds as normal.
States gets points for recognizing a trap.
Wade gets points for planning in the event of failure besides just running.
And player is given points, being able to break binds that stopped even Statesman and proceeding to force a genuine retreat out of Wade. -

Problem with that there be that many NPC factions don't have unique weapon models any more that can't already be gained in game. Closest would by that the Talons of Vengeance can use Scirocco's sword.

you know, it actually looks a bit like what some of the Seers wear. Given Penny's psychic POWER LEVELS and still comparatively little experience, it may be a limiter to keep her from accidentally destroying things in a panic or similar bad situation.
or it might help her focus her psychic powers into other abilities, like given it's attachment to her back, maybe it enhances her synaptic connections with whatever psychic foresight she might have to enhance her reflexes. -

Mostly for some additions to Beam Rifle/Pulse Rifle.
I think six new additions might be a bit ambitious, though. I'd settle for 2-3.
- Western Style, double-edged sword
- Super High Res katana with a different tsuba (guard) pattern with actual gaps in the design, maybe a detailed Habaki (blade collar), and a blade that has reflection tech applied to it. (or at least the sleak metallic 'reflection)
- Shirasaya style katana
- Odachi style. (Not quite Titan Weapons size, but definitely longer than the standard katana.
- Circle of Thorns, Thorn blade (because the handle is long enough for two hands)
- A giant, straight edge, meat cleaver shaped sword.
- Katana with various auras. (Electricity, fire, energy, etc.) possibly with two color options, one has an untintable hilt and tintable blade and aura, and the second being tintable blade and hilt but untintable aura. (since the devs no long seem shy about elemental weapons)

The problem is that I don't think Fusionette is in AE.
Nor is a high level Flambeaux. -
it's sort of an interesting list to read and look over it amongst my villains and heroes in comparison.
For example, a majority of my villains fall into "noble demons" or even just heroes on the wrong side of the fence. They're not out to cause problems for the world, they've often just lost hope that they can make a positive difference and instead have resolved to fortify their small lots in the world. anyone who's trustworthy and loyal can join into this lot and gain protection.
On the otherhand, my true villain is not all about killing randomly, but he is a huge troll in many respects, he pisses people off just because he can afford it, and despite the former mentioned about killing, he has no qualms about it, indeed, it's the end result of many of his objectives (mind eating). -
As far as Penny's costume is concerned, she's at least better protected from the elements than some OTHER psychics in New England climate that could be named.
*cough*SisterPsyche*cough*AuroraBorealis*Cough*Ser afina*cough*
Okay, the latter could be justified since she's not human, and as a friend of mine pointed out, they're all powerful psychics, so chances are they have a sort of personal climate control thing going on for them. Especially reasonable for Psyche who actually does have forcefield powers.
And Penny's supposed to be more powerful than Psyche, she wasn't so bad in the LGTF aside from being low on the hit point scale. So what are we looking at here? Is she going to be an example of being an Incarnate Equal without actually being an Incarnate?
Randomly, something I would have loved to have seen was for all three Clockwork Kings to merge into one and become insanely (possible extra emphasis on the insane) powerful force, not unlike the Dragon Ball Z Super 17 saga.

If Flambeaux has Energy Manipulation she'll have Boost Range as well...which gives her an edge. If she's Fire/Fire it's just more AoE goodness. As for KB, Flambeaux can just use Hover. We know she can use the Market since...*SPOILERS* in a Twinshot arc you speak with her next to Wentworth's, where she found some IOs to make her hair better. She might have enough KB protect from IOs to make that meaningless. -
I get it now! Penny's costume looks like it does because she's a fan of Testsuya Nomura, the dothack franchise, and likely other Asian pop culture shows.
Also, I kind of wished Penny had joined WISDOM, that's a group that seriously need more members, especially members that appear in-game, or maybe have WISDOM merge with the Phalanx, because seriously, there might as well be only two people in WISDOM right now since Rose Star and Spark Blade never even get mentioned in-game. -

As a random note: why does her description claim 'no one knows who trained her' it was mentioned she was being trained by Sister Psyche and Aurora in the LGTF if I recall. Though given her costume, maybe some people are right and she's starting to take after her mother.
edit: another interesting note is that a closer look at her costume also seems to show her taking some costuming cues from Maelstrom.
