Two is the number, the extra 2 you heard about was not for a boxed copy of the game, it was specifically for purchasing Going Rogue. Going Rogue came with 2 slots when you purchased it, and so you get to keep them if you had GR.
The thing about that box copy was that you got an extra reward token for it. While reward tokens are one per month GOING FORWARD from now, you only received one token for the 3 month veteran status you had before. You received one FREE token that everyone gets, and you received one token for the box copy (Hence why you have three reward tokens..)

I have a 3rd account which I created over a year ago for triple boxing with. My understanding of the number of slots available for Premium players was that you got 2 for a free account and an additional 2 for a purchased copy of the game. Is there a list I can refer to that details what confers extra character slots?
Currently my 3rd account is showing as 2 slots only. Has 4 months played time (though only 3 reward tokens) and was created with a trial + retail code (probably bought through the NCsoft store rather than a boxed copy).
