Dark Dark Dark build request
I can't offer a build, as I don't use Mid's and mine's a dark/sonic. But I can tell you hitting the ranged defense cap should be reachable, and it makes a lot of difference. I didn't worry about smashing/lethal at all, and went for recharge after that. Slotting Shadow Fall and Maneuvers with Red Fortune and putting Thunderstrikes in the single-target attacks, plus Lockdown in Petrifying Gaze will get you a long way. Plus, Stupefy in Dark Pit, maybe. And a set of Gaussian's in Tactics, as dark really falls over if it can't hit.
Being at the ranged cap keeps my defender alive, and all the to-hit debuffs I consider gravy. I prefer to be difficult to hit before I've launched attacks or debuffs, and even harder to hit afterwards, but you could easily make a case for moderate defense and letting the debuffs handle the rest. Not sure how well that stands up in incarnate content, however.

This doesn't cap soft cap, but, since D3 is all about -ToHit, you usually don't need to get there. Once Mids is updated to I21, I'll probably tweak it since you can take Epic Pools at a lower level and possibly six slot them, which couldn't be done before.

Ouch, I hate the build above. Twilight Grasp very poorly slotted, Tentacles poorly slotted for damage.
This build caps s/l/e using Scorpion Shield, you can do it without it but you won't manage to get almost permahasten levels of recharge like this without resorting to purples I suppose. Problem is, it still has room for 3 powers and has no extra slots left (actually not a big problem, I already took all the powers I like in both combos and kept your Flight).
This build also has a good 33% ranged, putting you at 1 small luck from softcap.
Btw I'd put Fearsome Stare at 12, it's at 35 because I forgot about it (headache):
EDIT: headache over lol, so edited the build because I'm a n00b and put the kismet in Vengeance, also to fix Fearsome's placement and I like recharge on Howling Twilight due to -regen so I took one slot from Stamina:

One thing NOT to do is place Kismet +6% in Vengeance ... Kismet only functions while the power is active, so ideally a power taken at low levels that you intend to run 24-7.
What is your goal with this Dark/Dark/Dark Defender, NightPrincess? Permahasten and defcap with a combination of +Defense powers and -ToHit?
That's the only reason that I can think of, that would explain why you would do things like 5-slot Doctored Wounds into Twilight Grasp, instead of using 5-slot Touch of the Nictus, the wasted accuracy enhancements in Tar Patch and Howling Twilight.
To answer your questions, no. You don't really need to worry about the defense cap. You'd need to worry about it even less if you buffed your -ToHit more. I mean. Seriously? You'd rather buff Fluffy's hold, instead of buffing his Chill of the Night aura, his Tenebrous Tentacles, and his own copy of Darkest Night all at the same time?

Some further thoughts:
Not sure I'd go for soft-capping ... 32% or even less is probably very survivable, especially teamed, given the amount of AoE -To Hit debuff you can generate between Darkest Night and Fearsome Stare.
I think both Tactics and Tar Patch are eating up too many slots for the return investment.
Fearsome Stare (as Kioshi pointed out about his build) is too late in the build ... if not an ASAP power it's durn close to it. Obviously if this is a level 50 respec build (which I gather it is) the level at which you gain Fearsome Stare is only truly important if you exemplar frequently (which would be true of your power order in general).
I'd rather see Tough (and to a lesser extent) Dark Embrace slotted up more, probably a minimum of 3 slots a piece.
If you plan on exemplar status often ... use level 25 LotG +7.5%, a level 10 knockback IO and a level 10 Steadfast resist/+3% defense. At the very least these IO's should be at or below the level you expect to exemplar with any frequency.
I'd try to fit that Kismet +6% in Hover or Shadow Fall.
Oh yeah as I said, Fearsome Stare is a level 12 pick. I put it late in my build because I forgot about it. You could move Boxing to 18 and P. Gaze to 35, I put it there for the sets, I never found much use for it, you have so many good tools, plus you won't need a hold early on with Fearsome Stare's great radius and range. It can be useful in the build I made because you can stack it with Web Cocoon for bosses, but you picked another mastery so it's kind of 'meh' imo, but maybe you like using it, I don't.
As Doomguide said, Kismet is bad in Vengeance. My bad also because I put it there, I usually put it in Combat Jumping, it was kind of an afterthough 'damn I can't put more lotgs lemme put a kismet here then. Damn headache didn't make me think good lol.
I'm not sure if I would softcap either, but it sounded like a fun exercise. I don't have an IOed Dark/Dark, just an old SOed one I shelved. And except for the LoTGs, I found it surprising I could get so much rech and still softcap kinda 'on the cheap'. I think it could pseudotank the ITF with the over the cap defs and all the to-hit this combo provides.
Oh btw, look up Silas' guides, they're kinda current and in his guides there is one for every support set.

After returning from a long break from the game i find my Dark Dark Dark defender is lacking and i guess i'm going to have to find a good build. The only power i'm dead set on having is fly.
Is it possible to get close the defense cap as Dark Dark Dark?
Do i need to even worry about the defense cap?
