Looking for some guidance...
Join the main global channels in the game JFA2010 or Justice United. It makes it easier to find teams, get questions answered, etc. I'd recommend stating upfront that you've been gone for a while and that you have questions and ask people to send tells if they are willing to answer your questions. Use the tell system to ask your questions and get answers without flooding the channel. Some can get annoyed when the channels are used in that manner and they can't see team requests/info. Also, you could ask them in the Help channel instead. There are several people willing to help that monitor that channel.
First off, for the Incarnate system, you'll first need a code for the purchase of Going Rogue applied to your account. Without GR, the Incarnate content is inaccessible. The introductory Incarnate arc doesn't require a team, but some need help completing it. The rest of the Incarnate content we currently have is the new TF's and iTrials which do indeed require teams, if not multiple teams in the case of the iTrials.
Primal characters (started as either a Hero or Villain) can get to Praetoria through Pocket D entrance into Studio 55 IF they have purchased GR. Praetoria is a lowbie zone with 1-20 content. IF you have purchased GR you can start a new character in Praetoria using any AT (except for the EATs and VEATs).
The side-switching is pretty much as you list it. You can continue all the way to the opposite alignment as well, not just stopping at Hero or Villain, and there are missions that give a reward for staying a Hero or Villain a certain amount of time.
If the game spit out 20 dollar bills people would complain that they weren't sequentially numbered. If they were sequentially numbered people would complain that they weren't random enough.

I'd first of all suggest you join the main Globals on Justice. They are Justice United, and JFA2010. Although there maybe lulls in the conversation, they're both great spots to find teams, Task Forces, trials, etc.
A great source of information about the game can be found at the CohTitan Paragonwiki site. Sometimes the searching takes a bit to find exactly what you're looking for, but it's still fairly well laid out.
Here's a link to the Incarnate Abilities that we currently know about, and how they function. You will need to have purchased Going Rogue(GR) to access the incarnate abilities, and I believe the trials as well. You will also need to be level 50 to access Incarnate abilities and trials.
You'll also need GR to access side switching via Alignment Missions, aka Tip Missions. Tips will be found under contacts and they will randomly drop after you defeat enemies Lvl 20 or higher. The tip missions will give you the opportunity to choose to change your alignment. You're only allowed to do 5 tip missions a day for progress on your alignment change, with the Morality Missions not counting against those 5. After you've done 10 of a specific alignment missions (Aka Villain, or Vigilante for example) your next tip to drop will be a Morality Mission. Here is where you will make the choice to change, or keep your current Alignment.
The circle of Alignment is Hero>Vigilante>Villain>Rogue>Hero, starting at either Hero or Villain. Vigilante's are still considered Heroes by the game, but can access red side zones, with some limitations to starting content. Rouges are still considered Villains, but can access blue side zones, with the same kind of limitations.

Like stated above the 2 main global channels, and if u do join them ask a MOD of said channels to unsilence u if they have not.
Yup, Going Rogue already purchased and on my account. I'm unsilenced in JFA2010, and I'll get in to Justice United tonight. I'll need to get a team for the Incarnate unlocking chain, there's no way I'll be able to beat the Honoree/Vanguard mission. Two EBs at once = dead Doc.
Now I need to figure out if my SG is going to be active at the same time as me, or if I need to search out a more time zone appropriate pasture.

3. A lot of people are running one man SGs as of late the more I talk to people so Its definitly not unheard of.
The only reason I am running a 1 man SG is because all the other folks either quite or playing other toons. But I do have a couple of others in my SG that do play once in awhile.
So, despite being away from the game for almost 3 years, I'm still in my SG. Great, people to play with!! Nope, nobody seems to be on when I am. Which is a problem, first and foremost because CoH is a social game, and secondly because I have no fracking clue what it is I'm supposed to be doing!!!
So, I hear there's some endgame content /advancement from the Incarnate system. Working on that, looks like I need a group though, my little blaster can't handle to EBs at the same time. Also, am I supposed to be able to get in to Praetoria, or is that all lowbie stuff? And either way, how would I get some of my existing lowbies IN to the zone?
The one thing I have figured out is the Vigilante/Rogue stuff. Go do the little missions, do a bigger mission, get access to some villian zones. Although I'd imagine there's some reward for being a pure hero or villian... Meh, Doc Amp has an arch-nemesis that needs tracking down on the Isles anyway....But I digress!
