Dump the KB resist steadfasts. Get a 3% def steadfast instead, put active defense on autofire instead of hasten.
You weren't soft-capped to normal content... and incarnate trials have enemies with 14% higher base to hit.
I took the liberty of revising your build a bit. The new build is quite comfortably softcapped to ranged and melee, but not to AOE. Maybe somebody who's better with shield than I am can do more with it. Personally I'd be a little concerned about the endurance consumption here if I were you, but you've got some extra end management powers... still I'd probably turn off tough a lot, myself.
A barrier or rebirth destiny, or working in aid self, would help you a lot too. When you trial as it is, I'd say carry 2 columns of purples and 3 of greens to cover yourself with. Maybe switch to 3 of purples and 2 of greens.

Hmmm, a few good ideas in there.
The Gaussian set would help, as well as the Touch of Death. I'll probably drop 2 of my slots in Fly and 1 in True Grit to make soom.
I don't really see what the Red Fortune's buy me though.
Replacing Invis with Manuevers? Hmmm. Both are camels for the recharge LOTG and the invis does help in ITF speed runs.
Losing the knockback protection isn't an option, since I do enjoy an occassional Hami raid.
Now, I just need to figure out where I can fit in the Steadfast 3% defense enhancement.
As far as endurance usage - never been an issue.

Phalanx Fighting is not enhanceable so it should only have a single LoTG +7.5% recharge or Kismet +6% acc slotted in it. I replaced Fly with Maneuvers to reach the soft cap of 45%. Swallow one small purple and you'll be a couple ticks off of the new trial soft cap of 59%.
You didn't indicate if you have an Alpha Incarnate power as it will affect a lot of the slotting. I recommend Cardiac Core to save on endurance/recovery and a bit more damage resist. You may be able to drop Physical Perfection and take Fly again if you do.

Red fortune x5 nets you a 2% damage buff and 5% recharge buff from set bonuses. I didn't see anything particularly better to do with those slots, off-hand.

I do have an Alpha incarnate - the tier4 recharge one. Are you sure about Phalanx not being affected by enhancements? It is an auto defense power...

It's a great power to have as it's auto and helps you a lot when you and your team are dog piling an AV.
I will check your build out again with the Spiritual Alpha a little later. Is it the Core (45%) or Radial (33%)?
Phalanx fighting adds 3.75% def for up to 3 teammates who are within an 8' radius (With you counting as one teammate), so a maximum of 11.25% def. You can slot it with 6 def IO's or leave it empty and the numbers will still be the same.

Here's a revised build but it's not far off from the other one I recommended. Slightly better HP and regen but worsened endurance; better damage but slower recharge.
Left the KB protection IOs but Active Defense provides 10 pts already, so you currently have 18 pts of KB protection. That would take like 4 Nemesis staffs hitting you simultaneously to knock you over.

Try on this MA/Shield. It's what I'm building for, with two sets each of Touch of Death, Mako's Bite, and 5-slotted Scirocco's Dervish. Throw in a full set of Gaussian's into Build Up (it IS overslotted, but that's a sweet defense buff) and you're pretty much softcapped. Throw out Combat Jumping for stealth, and go hasten/SS instead of fly, and viola! Similar build to yours, with more positional def, and I think an extra attack for Crane Kick (just no extra slots). Went with Red Fortune for 'cuz it's cheaper. Didn't slot Aegis (five slots is what you need to get the most OOMPH from that IO set. So resistance may not be to taste, but then again, if you're not hit, who needs res?

I've been fairly happy with my MA/DS scrap - until the incarnate trials. My defense appears to be totally ineffective even with +2 level shift.
