Switching Power Trays When Zoning
I have noticed this from time to time, but the fix for me has always been to press the left ALT button once without touching anything else.
After that all the keys work as they should. It is almost like the keyboard value for ALT got left in the shifted position by mistake?

I agree with this. Many times, people use the zoning as an opportunity to Alt-tab to a different application. The problem (as you have noticed) is that the game knows you pressed Alt, but once tab is pressed and the other application has focus, the game never knows you ever released it and thinks you are still holding Alt when you switch back. As mentioned, pressing the Alt key again will make the client realize that you are no longer pressing the key, and things should act normally again.
Sometimes (and very infrequently) when I zone, the number keys activating my powers in Tray 1 (bottom tray), begin activating my powers in Tray 2 (second tray up from bottom).
The fix is relatively easy, I switch Tray 2 of powers to be the bottom tray, and Tray 1 to be above the bottom one. All keys work normally at this point until I zone again, then I have to switch back. It usually doesn't happen again in the same play session.
This has been going on for a few years, but it's intermittent, and not that big a deal so I haven't worked hard at tracking it down or replicating it.
Things I have checked or noticed:
It happens on different computers
It happens with different keyboards and mice
It happens with different OS's
It's only the lowest two (first 2?) trays (well frankly I click on anything not in those trays)
Keys, both number and Alt+number work fine once I switch trays
No one I've mention this to in game has had the issue.
