Ooops, somebody forgot to put something on the model! 
I presume you /bug'ed the problem?
Maybe they saw you coming and were trying to be funny? "Hey, the team leader's name says 'no body'. Let's send Joe since he's got no head."

I'm guessing the boss needs a new name "Headless Wailer Lord". Bugged this last night but when help arrived they were convinced it was a mish complete issue despite our efforts to "point" it out... oh well at least the OP got some good "head" shots... ha..... hahahaha.

I was teaming with 2 People i team with a lot and was running a Story arc Old School and found the boss for the mission was missing his head, 3 Screen Shots to show...
Not a big bug but i think it was funny and need to show.
