Short answer is that player attacks have a base hit chance of 75% in PvE. This is further modified by the accuracy of the attack. Most attacks have an accuracy of 1.0, while many weapon attacks, like Katana, have a modifier of 1.05. You can see that using the in-game numbers.
PVE tohit starts at 75% as a base if you're attacking something even-con (white). Some power sets include a 5% bonus.

Wow thanks for the quick replies. I read a lot of stuff about defense and saw the 50% to hit base for critters and thought it was the same for players. I guess I should have done my homework (no wonder I kick the crap out of minions my level without using enhancements).
So, just to clarify (I'm not the brightest), Mid's is doing the final "to hit" calculation and just calling it Accuracy?

And if you read the Paragonwiki links above, you'll find that your hit chance is capped at 95%; Mids' does not cap the displayed value. However, the value goes down as you face opponents higher level than you, or opponents with Defense buffs or ToHit debuffs.

And it's assuming you're fighting enemies at your level, who don't have Defense powers of their own, and aren't hitting you with -ToHit debuffs.

I downloaded Mid's Hero Planner and it is AWESOME. However, it has created some confusion for me. When I look at say a Scrapper Primary, the "default" accuracy is 70 something. Where does this number come from. I was under the impression you start with no accuracy (or a multiplier of 1) and get a little boost at the start for certain skills (like 5% for Katana). What am I missing?
