Boss bugs?
I've been duoing with my wife on middle difficulty setting for a few months now. I can't say there's much difference in the number of bosses spawned. Quite often 2 per mob. This blue side Defiant doing story arcs.

Me and Blue Rabbit managed to get spawns of single, +2 bosses in a mission despite, as he insists, the difficulty being as low as possible.
I suspect he's a fuzzy little liar, but we weren't having any problem getting bosses!

Nope! Was a bunch of Lost though, it's troublesome keeping him alive at lower levels, let alone with psionic damage.

I was on Villainous! I swear! And you can check it yourself by opening the mission's window and seeing what reputation is set for the owner of the mission.
It's just that my Fire/Psi Dom is very squishy in the young levels...

I was on an Invincible Manticore TF with my scrapper yesterday and that consisted almost entirely of Paragon Protectors, so bosses will spawn in large amounts sometimes.
Yes just try a high difficulty Malta mish on a squishy - you'll have no trouble noticing the bosses especially the Gunslingers. Trust me sappers aren't the problem.

This sounds more like boss spawning has been made more random to me.
Two player team getting bosses on low diff, 4 player teams on inv not getting bosses at all...
...I'd say something's slightly (insert generic strange word) here...
Edit: I just did a regular paper mission on 3rd diff, two man team:
Not a single minion, not a single liut, just pariah bosses (Lost glowie mission) one in each "mob" (that is, one lone poor sucker in every room)
Seen that a few times - not too bad for single target toons but a real pain for AoE builds.
Ambushes can be horrific at times - I have been hid by one of 16 - mostly Bosses and Lts and that was solo on heroic in the past.
Have also seen redside some huge mob sizes solo at high levels - OK I took 4th diificulty as I was playing an AoE based toon but mobs of 12 with repeated ambushes of the same hurts on a squishy

Just got another contact mission, lost again and bosses only again.
One the bright side, at least the game isn't as predictable as it used to be, but I'm still pretty sure it's unintentional.
A few patches down the line bosses seemed to almost disappear. Several of us reacted to that.
Now, I hardly play the game anymore, so I'm not totally sure about what it's like after the last patch, but I've tried several different constellations from 2 man low diff radio missions all the way to 8 man relentless/inv. contact missions and it still seems very very hard to get the amount of bosses we used to have.
I might be wrong and it might be intentionally from the devs. if I'm wrong or just unlucky, fine.
If not: With the game turning rediculously easy I guess it's one way of slowing down levelling, but what was the point with the "new" xp curve then? Simply put, it's dead boring to play the game when you solo or play small teams meeting almost no resistance at all, stumpling upon an EB every now and then. It's also somewhat inconsistent have such an easy ride then running into said EB getting PWND because you've stopped paying attention!
