If it goes in health its always on, and if its in a toggle its always on when the toggle is active. if you slot it in a click it will activate every time you activate the power.
Auto powers like Health actually have a cycle where they reapply themselves before the power wears off. The cycle varies but according to Mids' at least, Health's buff duration is 10.25 seconds, which suggests the power probably reactivates every 10 seconds.
So, every 10 seconds, the Miracle buff will reapply itself with Health. Every usage of Transfusion will also reapply the Miracle buff (although it won't stack it).

If you really want to or desire every bit of regeneration you can get. To be honest though, the difference between two and three level 50 gIO enhancements is not very much, so if you need the slot elsewhere but still want a strong effect, just two Heal gIOs should be sufficient.

If you use max level IOs you can slot Miracle Heal, and the unique, Numina Heal and the unique, and get pretty close to the ED softcapped bonus for regen AND the benefit of the uniques AND the first set bonus in each set all in 4 slots.

Milady's, that sounds pretty clever. I now have the Numina unique slotted in transfusion for a total of five Numinas there. I have been considering going to 6 Numinas there for the Ranged Defense bonus... but I could then go to four instead and move the Unique to Health?

Correct. And the Regen from the Numina's unique is slightly more than you would get from the Enhancement bonus of a lv50 Heal generic IO.

If you bother with that (I figured, slot what you have, figure out what to do if you get the extras), you may as well go for the gusto and stick a Regenerative Tissue in there, either replacing the Miracle:Heal or adding another slot. Either would be adding to the total.

What AT are you? For most AT's, in my opinion, it's a waste to slot 3 heals in Health. Certainly, for any AT with even the poorest self-heal, it's really a waste.

You're aware that Transfusion has to hit the enemy, right? Most people slot some accuracy in it, or an Accurate Healing set. Of course, if you have a ton of global accuracy bonuses, no problem.

Yeah, accuracy is not a problem. Last time I checked I had a 73% global acc bonus.

I usually slot Touch of the Nictus on lower kins
For a Fire/Kin just slot the 2 uniques in health and save the rest of the slots. Regen on a controller is not the best use of your power slots unless you went with Stone Epic and can perma Earth's Embrace.

I mostly use the firekin for farming, so regen is actually pretty useful as I am normally tanking, at least in the old-style farms in PI. In the AE, I wonder if any firekin (even purped out) can really handle himself without a tank for help, but now I am getting off topic :-)

So when does the 120 seconds start?
What if I slot Miracle: Recovery in Health, as I have seen in some builds? When do the 120 seconds start then, as Health is always on?
