First off, Peacebringers and Warshades are Kheldians, but Peacebringers are not Warshades. So, what you've built is a Warshade, not a Peacebringer even though you mention starting out with a PB.
As for the build itself, there's nothing wrong with it, however, I would leave Nova slotted with just EndMod and use the slots elsewhere. I would also pick a different slotting for Dark Extraction, one that would emphasize their recharge over their damage because I think having two pets is usually better than just one pet doing more damage.
Unchain Essence could use some love, especially since coupled with Gravitic Emanation it makes a good tool for stunning Bosses.
Eclipse can be turned into an Endurance recovery tool and since each undefeated enemy around you contributes to your +DmgRes, having a higher +DmgRes slotting in Eclipse isn't as important as it is for your shields or for Dwarf. Speaking of Dwarf, you could frankenslot it for better +DmgResist and better EndMod/EndRdx values and this can make a difference in how survivable you are in Dwarf form.
Orbiting Death is mostly useless if you're not in Human-form enough to take advantage of it and when I am in Human-form on my TriFormShade, I usually find Inky Aspect a lot more useful than Orbiting Death. Finally, since this is a TriForm build with emphasis on Nova, having such wonderful recovery tools such as Stygian Circle and Eclipse, I think the Fitness could be skipped entirely, mainly because your Human-form attack-chain is not going to actually require you to stay in Human-form for long periods of time.
On my TriFormShade I find that I swoop enemies with Nova, hit them with the Nova blasts, followed by Gravitic Emanations, then a well-placed Gravity Well that usually defeats a Minion so that I can use Unchain Essence, Stygian Circle, Eclipse and Dark Extraction (whenever they're up) and shift to Dwarf for a fun melee-centric fight... not that there's much left to fight me, especially not if I mired my Nova before the fights.
Be advised, all of my comments are based on my playstyle and experience, and YMMV.

Honestly speaking, the thing I really can't get over about Orbiting Death is just how large an area it covers. It's much larger than the damage/taunt auras on meleers, and it always seems to surprise me when foes quite far away from me start getting those orange numbers.
Of course, it's also dangerous to use for just that same reason. I almost never use it, because I can't currently handle the aggro. It's just so cool, though, that I can't bring myself to drop it.
And just to mention, I have it on both my Tri-Form and my Human Form. My Tri-Form it's taking up space, I did not want to take any ranged attacks with him, so the choice was forced on me at 4. With my Human Form I have it on one build, but not the other. I suspect after I get my Human Form's shields slotted out and get Stygian Circle to handle the End cost, I'll be able to use it. But for now, on both characters it is unslotted and unused.
I just turn it on from time to time just for grins.
If you're a Human-only, running your shields, Eclipse, Inky Aspect and running with a pet or two, I bet you could handle things a bit better... but for a TriFormShade, it's more of an Orbiting Debt, or an un-used power

I took Fitness because I couldn't really come up with something I might want instead. Is there anything that'll add to my survivability/damage output more?
I can say that with 3 click heals (2x Human <1 being a Dull Pain clone> and 1x Dwarf), a 25 second recharging phase power in Quantum Flight and 2x 140ft Nova blasts slotted with multiple Proc IOs...I really enjoy my PB for PvP & PvE.

I think what threw me off is that you very smoothly went from talking about Peacebringer to talking about Warshade but if at all, referred to this as a change in build. Since I view them as two separate ATs actually, I got confused.

If you're not going for set bonuses from sets slotted in UE, I personally like to frankenslot it for Acc;Recharge;Stun and if possible, a dash of Dmg;Slow;EndRdx. It really depends on what you want to use the power for. I like to use it as a stun AoE that will stop minions from attacking me while I extract the essence of the defeated NPC, then use Stygian Circle and/or Eclipse if they're up. Others may use it for DPS, but the thing about Warshades is that since we increase our damage potential with those nice Mire attacks, it should be quite easy to use either Mire power and then defeat one NPC and use UE and see a huge chunk of healthy minion-level NPCs melt away, more often than not, they'd simply collapse with no HP left.
The hardest thing to get right about UE is the timing and position of when and where to use it. The slotting totally depends on what you enjoy doing with the power. Since I enjoy stunning NPCs, it's usually +Acc;+Recharge;+Stun, anything I manage to get above and beyond that is sugar-coating.

Increasing the survivability/damage output of a TriFormShade has to come from analyzing your combat tactics. The problem with pool powers is that if they're a toggle power, they're almost useless for a heavy form-centric approach because the power stops being effective a few seconds after you shift from Human-form and the toggle power deactivates.
Power pool powers that are click-powers are slightly better, but require you to be in Human-form to activate them, and their benefits carry over to the forms, but need to be refreshed whenever the power times out and its effects expire (i.e. Hasten).
Passive pool powers like Health and Stamina require our Kheldians to shift to Human-form before activating, and once we shift back to whatever form we require access to, the powers' benefit remains in effect for a few seconds still. The only benefit I can see to taking Health and Stamina (especially for a TriFormShade) is if you plan to slot them with the +Recover;+Regen proc IO's. I forget what those enhancements are called, but if you don't plan to get them, perhaps the shields or Human-form attacks would be more beneficial.

So, I usually play more damage-oriented characters. My first fifty was a Scrapper, and my second was a Brute. However, I sat down to try out Peacebringers about a week ago, and they just didn't quite click for me. So, I went back over some guides. I poked around here, and played around in game a bit. I've put together a build, and I'd like some opinions.
First up- it's triform. I putting a bit more emphasis on Human and Nova powers, because I don't really know much about tanking, so I'm going to save Dwarf form for an off-tank/panic button situations, or times where I'm teaming and comfortable with the current pace.
Other notes: I took Superspeed instead of Nebulous form. I think it'll give me better overall utility.
