Point to note. The Game Card is actually different from a time card.
With the Game Card you can create a new account or upgrade a trial. Its just like buying a boxed version, but without the disc.

The game cards are different from the game time cards. The game card ($20) gives you a game account, 1 month and a jet pack that lasts a month. The Game time cards (any NCSoft time card will do, I believe) just give you the gametime.
EDIT: You might also look at getting the Architect edition. I think that should also just be $20, you'd get the cd, 1month, plus one of the costume packs free. That can also be used to upgrade your trial account.

3) You're not putting an extra $5 towards the game, you're putting an extra $5 towards Target. The retail outfits bump the price up in order to make a profit off them.
EDIT: thought the OP meant a time card, my #3 answer may be incorrect in that case. I wasn't aware there was a retail version that had teh game but no disc.

The 20 dollar cards are a special type of card. Those cards get you access to a limited-duration Jet Pack temporary power, granting your character flight while it's activated. That really is the only difference -- you're spending an extra 5 bucks to have a travel power for any character at level 1.
15 dollar cards exist, but damn me if I can't find them anywhere, though I mostly restrict my shopping to Wal-mart, which will probably change soon since Wal-mart never has the games I'm looking for when they come out or have "gone out of style". (I'm looking at you, Orange Box. Or not, as this case turns out to be)

Here is a suggestion. I do not know how old you are but if you have a checking account that offers a visa/mc debit card, you could use that instead of a normal credit card. I use my debit/bank card cuz it has a master card symbol on it. The cash comes right from my bank account thus no credit card debt from gaming.
If you are under 18 and have a teen checking account, you may still be able to get a card but that usually means you need your parents approval.
If your over 18, have a job and a checking account, go to your bank and ask for a debit card that has a checking account symbol. My bank actually gives me cash back bonuses for using my debit card as a credit card (its only 1% but still thats cool).
