Buffs can sometimes vanish from your buff bar temporarily due to lag or a very large number of buffs being on at once. The buff is still affecting you, it's just not being shown. Most buffs will still show up in your Combat Attributes window if you open that, but I'm not sure if XP buffs are shown there or not.
XP boosters should work at any level, with any team.
I've never had an XP booster go straight from email to being 'on', though - they've always shown up in my powers list first, then I put it on my tray and click it when I want to start it. Once active, the power vanishes from your powers and power tray unless you had multiple charges of it. After that, it should show in your buff icons.
Also, I've never used any of the 2-hour ones, only the 1-hour 25% ones.
It's possible you encountered a bug. I doubt this is any sort of known issue.
As per recent redname posts, XP boosters will now be immediately activated upon being claimed from email. This unnecessary and undesirable change is intentional.
The email-to-active part is intended, and was changed in the last patch. When you claim one, it gives you a pop-up telling you how it works, and you have to click OK before it goes into effect. Subsequent boosters might not give the warning, or there might be an option to not warn you about the immediate effect (similar to deleting enhancements and such).
As for the duration, that might be a bug. The warning does say 2 hours, but before the last patch, they were 1 hour. As Blue Fenix mentioned, sometimes the buff icon doesn't show up, but it would still be in effect.
Also as mentioned, there's no scenario in which an XP booster wouldn't work.
I have used the XP Boosters on live and they also were 1hr(I think that's the correct duration.). There are a slew of new XP Boosters on the beta market; XP Boost of 1.5 or 2x(instead of the 1.25 on live), and each XP amount has a handful of different durations available(from 1-8hrs I think), whoever wrote the text for the confirmation window may have confused the "old" XP Boosters with data from the new ones coming if it said 2hours.

I claimed an XP Booster from email last nite, it instantly went to "active" and then I teamed my new BM with the wife and we ran some missions. After about 20 mins, I noticed the booster icon was no longer on my screen nor was it listed in my powers. I claimed another from my email, but this time got a pop-up telling me that this XP Booster would last for 2 Hours and I clicked OK. After about 45 minutes, this booster icon was gone and no longer listed in my powers. Tried to claim a 3rd, and this one did not give the pop-up warning but instantly went to the powers tray with the description of "1 hour of Xp Boost".
Sent in a ticket too, but wanted to post in case this is a known issue, or get feedback or suggestions. Maybe the XP booster is not supposed to work on a low/new character teamed up with a 50 ??
