Step 1) Have the leader log out and back in. If that doesn't fix it....
Step 2) Have EVERYONE log out and back in. I've never seen that fail to fix it.

Hmm, we got exactly the same effect today on a Synapse TF. We tried relogging the leader to see if the new leader was recognised by Synapse, but to no avail.
One of the vets on the team remembered an old bug that is caused by the Zombie Apocalypse event happening when you're running a task force.
We went and killed zombies until the event ended, then redid the mission that was looping and it completed correctly and the TF continued, so it seems plausible the event was the cause of the issue.
