Just chiming in to say I had pretty much the same thing happen, though on redside and thus with the St. Martial Facemaker rather than the F.F. Icon. I also handed the final mission in after claiming the halloween salvage.
From what I can tell, it is the level 40 costume mission that is bugged. I had the same happen. A petition was sent, and after investigating it (and bumping it up the line), they awarded the slot.
Have an update on this one... known issue, and the problem that prevents us from getting it ingame, also prevents them from manually giving it to us.
Fix is due next week.
Send a /petition telling them about it, so that when the fix is applied, they can manually review everyone who has told them about this, to see if the problem has been resolved
*edit* I posted the reply before i saw the previous poster... interesting, but it looks like somehow I managed to mess something up so that they couldnt manually award it to me :S

My GF had the same problem unlocking the 5th slot (already used the salvage). The support ticket said a patch was coming, and slots couldn't be manually opened before then.
Loose --> not tight.
Lose --> Did not win, misplace, cannot find, subtract.
One extra 'o' makes a big difference.
This isnt about the purchasable slots, but about the ones that you can earn ingame.
I got my character to 50, and the 1st thing that i do is the Tailor missions and the Halloween salvage hand in.
BUT i only have 4 costume slots available and not 5
The only thing that i can think of that *might* have been a problem was that i handed in the founders falls tailor mission after i handed in the halloween salvage mission.
Bleah, and now all of them wont give me anything to do to get the 5th costume slot.
