I will ask some questions, hopefully they make some sense at best they might induce some thoughts on either parties.

Why have you frankensteined zombies set bonuses? Would a full bonuse to defence from complete blood mandate be useful aswell?
Twilight grasp has maybe little too much slots, I would personally shave one off and use fivepart Nictus, but mileage may vary.
Two slots on Howling Twilight? How often you really need it, maybe slot it with one. Living thin is the way for those loving zombies.
I personally slot tar patch with frankenstein pace of turtle and Tempered Readyness slow/rec/end and last slot with pure +slow. With enough recharge abilities, you can pretty much keep ground patched fine. I dont think you need five slots there, but just my thoughts.
My Shadowfall is slotted bit like yours, except with Luck of the gambler and your spethial defence thingy.
Get rid of gloom, all necromancy damage abilities are utter refuse. If you want to do damage, get a ugh, temporary power gun or get veteran reward sands of mu. you will mostly try to cast dark miasmas healing nonstop and add in between crowd controlling spells. Dont waste five slots for power that does not damage properly, its waste of your skills.
Eventually slot the purple pet damage set, full for Grave Knights.
Use Soul extraction either as you do, or skim it down to only two slots and use it to 'bag' just those resistance and defence uniques. They affect rest of your undeads even if soul isnt extracted.
I used my lich to hold def and resist bags you hold with your soul extractor. Then dual add minimal bonuses for lich for acc/dam from several sets as you wish to frankenstein, the cloud stuff is good there, as lich is an aoe attacker with its powers.

Lose Black Hole, install Hasten with two slots, slotted for recharging and keep it on autocast. It should pretty much perma Dark Servant as your pet with setbonuses.
Unless you really need set bonuses from tactics, consider if its wise to spend to it four slots. Just slot it with two and endurance reduction and spend rest to some other power or possibly set which gives superior or same bonuses with little extra push.

As it looks like your going for defence build, focusing on Scorpion shield.
Some of the powers you dont fancy much or use constantly, switch attention to fighting tertiary pool. Get, donnow kick/punch, toughness and definately weave. Slot weave for maybe minimal defence, but you can with it, comboing it with scorp to raise your defence further.

Another question would be what usually seem to kills you in the missions or out in the world where horrible heroes prey on innocent... hmm working things?
If you need seriously status resistances ponder how to get them, you could go with combat jumping and that path, its thinner in endurance hog way, but seriously when you hit incarnate your pbaoe status resistance field will nullify most of things hurting you. Or guess one would be easy going also with defence builds.

Zombies are frankenslotted fully for Max Acc/Dam, with a twinge of end.
Additionally to get out the two pet-def/resist procs.
Some of that is because if it has to be re-summoned, it's likely that it's a zombie. Some of that is also because end-rec/damage/accuracy are all that matter to zombies.

Twilight is pretty much set up the way that it is for the set bonuses that I end up getting from it, and because maximum accuracy has helped me a time or two with some of the more irritating elite-bosses/arch-villains.

Howling Twilight is double slotted so I can use it as a stun, on x8 mobs, or a -regen/debuff on the larger mobs like GMs, as it stands I can perma-use it on them, which is a very very nice thing. It shines as more of an offensive tool, then it does as a resurrect. (And it's not a shabby resurrect.)

Gloom is actually ... more useful then you think. Especially with the recharge, +HP, and +End Max of it's set bonuses. It ceases to be rubbish with the damaging proc for the interface, and the musculature radial paragon. I generally ping your average thing for 150-200 damage and when I don't need to heal something, I'm usually hitting something with it, followed by a black wand or Nemesis staff. Minions die to two of the three, LTs usually die from all three, and it's another -7ish % to hit for me to chuck around.
When I am healing, it times in perfectly between twilight grasp.
Still. You do have at least a partly good point, I'll look around to see if I can find anything that has some good set bonuses or, if there's anything else I can shoe-horn in instead of it.

As for soul extraction... If you're only using it for a mule... You're missing out on a lot. Zombies/Dark is fairly ... weak ... at ranged things, and the pet bug has one using /releasepets quite a bit on mission entry. ...And that lets you put out another somewhat damaging pet from the get go.

Tactics: You're probably right about that being able to work with less slots. I like the set bonus of the accuracy, since well... Twilight grasp requires that, but it also helps the zombies on the higher diff levels, what with them being -2.

Black hole... I don't use it. Ever. I'd like to use it, and would use it when I get ambushed from behind... ...But uhh... I never remember having it. It's just sort of sat there since the third age. Alone. Unused. Unloved. I've considered replacing it with group fly... ...Or with something that has -fly, but I don't need any more recharge. Dark servant is perma, as is tar patch.

As for what usually kills me in missions... ...RL Distractions, pet AI issues, nemmies, and ambushes.
Most of the time I can just sit there and wait out being held on +0/x8 because of the dark servant, and zombies being aggressive little buggers, the issue would be carnies, and Master Illusionists chain holding me... ...Break frees though.

In the end, I don't really have any glaring issues with the build, outside of the side facts that pets are flippin' crazy, and the Zombie AI is no better then the other AIs.
Flying mobs are irritating because of zombies being short ranged, and quite willing to stand there like idiots, rather then cycling ranged attacks but... (That's why we have the nemmie staff and the blackwand.)

There's not a lot I'd change (I disagree with a lot of Creamcat's post) and a lot of that build is similar to my own Necro/Dark.

A few minor suggestions though:

Swap the Glimpse of the Abyss in Fearsome Stare for Cloud Senses. This isn't a 100% definite better slotting, but personally I find that Fear has a decent duration unenhanced to the point that unless you're fighting deep purples you can pretty much perma it. So I like to combine that with an even bigger -tohit debuff. It's kinda a personal preference thing though.

If you're never actually using Black Hole may as well swap it for another power that could at least be an IO mule. Good option for that is Combat Jumping - low cost, decent benefits in its own right, and can take a LoTG +rech.

I'm not sure why you have Acc/End in Tar Patch? It's autohit and not especially endurance intensive (esp. when you have two End/Rech/Slow already). I'd suggest just moving that slot elsewhere.

I'm also not sure Hover needs the Endurance reduction IO - I'd probably move that to one of the pet upgrade powers, or replace it with another End/Fly (Zephyr) if you really feel it's burning endurance.

I don't think Tactics is overslotted, but I would switch it to Gaussian's for (imo) better set bonuses.

Personally I went for more -tohit and +heal on my Dark Servant to further eliminate incoming damage - the only proc I kept was Cloud Senses since it'll fire off everything except the hold. But that's probably a personal preference thing.

I do agree that Twilight Grasp is maybe a bit overslotted. But more than that, you've not soft-capped the healing value. All my Dark Miasma characters (I have 4) are currently using the first 5 pieces of Touch of the Nictus and I rarely have any issues with it missing - you still get a good chunk of accuracy from the 5 pieces, and then accuracy set bonuses from elsewhere in the build (and +tohit from Tactics in your build) so I don't think the extra is doing a lot for you. I could see you keeping it to 6 slots for the extra recharge though - maybe change the generic Acc for the Nictus Heal piece. If you really really want that accuracy - though I don't think you need it - change the generic Accuracy for the Thief of Essence Acc/Heal, due to ED softcap you'll still get almost as much accuracy (about 4% less) but gain ~20% healing enhancement.


Some of the above is just personal taste, and I don't pretend to be the most hardcore minmaxer out there so there's probably better tweaks available, but a few of those I definitely think would benefit you.
