"flagrant error" does sound more like a joke, though...
Nemesis automatons with missing text is a running joke. You WILL see it a lot if you do Nemesis missions.
Also, FLAGRANT SYSTEM ERROR is one of many pop culture and meme references that the devs like to slip into the game.
