Looks like you're running Stardock.
Stardock can do strange things to your system in regards to graphics (since it's piggybacking on the graphics drivers for some of its effects).
The fact that you've been hand-tweaking the registry probably isn't helping either.
Try disabling Stardock. See if that helps.
Then start in Safe Mode and try tweaking your settings again.
If not, due to all the hand-modding, my next suggestion would be a clean OS install.
Drastic? Yep! But unless you've kept a VERY careful log of what's been changed from what to what and when, you don't have a hope in hell of resetting it to default.

I tried to run the game in SafeMode, but ran into a problem. The NVidia drivers wont load in SafeMode, so no luck running the game in SafeMode. Now I have three versions of the game on my PC: regular, Test and Beta. The Test version has some of the same problems, but in it I can at least open the Graphics Options tab. Try to fiddle with anything, and the game freezes. I also have the whole cant quit without freezing problem with the Test version. Beta version, on the other hand had a few different settings. Most notably it had only 2x FSAA, and all the other settings turned all the way up except for Ambient Occlusion which has Slight Occlusion Strength, High Performance Ambient Resolution, and Fast Blur. This copy runs flawlessly while actually in the game, as does all the others. However, even my Beta version has the quit-freeze problem.
All the software that I can think of that would be interfering with the game, I have running on my PC I have had for pretty much the whole time I have had CoH (nearly 7 years). Yes, there have been a few updates to those programs in that time, but after excluding CoH from what those programs do, the game has run flawlessly. My problems truly only started with Going Rogue, and Ultra Mode.
I may have found a bit of a solution to some of my problems.
Since my Beta version lets me fiddle with the graphics settings (except for FSAA) I went and hand edited my registry for the Regular version to match exactly the registry values of my Beta version. I can now fiddle with the settings in theh Graphics tab! YAY! (well, as long as I don't try to turn up the FSAA.)
Oddly, now I can turn on and off the 3D sound in my Regular version, but not in the Beta version...
I still have the 'quit and freeze' error, but I think I can live with that. I just wish someone official could tell me why the game is behaving this way and what I could do to fix it. I have more than the recommended hardware necessary, so it's a bit frustrating to have these things pop up after such a long time with the game working so flawlessly for so long.
I have the "exit crash" too, I think a huge portion of the playerbase does. Last I heard the devs were publicly ignoring it, probably until they can tell us they figured out what they're gonna change to fix it.

Well, I tried turning off my "Stardock" but that does nothing. Incidentally, "Stardock" is the company, just like Paragon is the company that makes CoH. The Stardock software that I run is Windowblinds, Deskscapes, and CursorFX. Of those programs, the only one that has in the past given me troubles with the game is Windowblinds. And that problem was easily fixed by excluding CoH from being skinned with Windowblinds. It worked that way for years. And the latest version was worked on hand-in-hand with MS to make sure it would work flawlessly with the newest version of Windows.

CoH is not a product of Microsoft and doesn't use Microsoft's DirectX graphics tools. It uses OpenGL. Assuming that because it doesn't necessarily interfere with basic desktop display functions and DirectX-tied graphics doesn't mean that this is correct and true for OpenGL (a path Microsoft desperately wants to be thought of as "legacy").
As noted, in the past use of various pieces of Stardock software have caused stability issues with CoH and turning it off can relieve the issue in some cases.

On a second read. And from the followup post by Texas Justice it sounds like you were going into WINDOWS safe mode.
This isn't what I meant. I meant use the CoH safemode.
Open the NCSoft Launcher, right-click on the game and select Properties. This window will come up.

Check the checkbox and click OK. Then start the game. This should completely blot previous settings.
From there, re-adjust the settings in-game and go from there.
Then, when you're done, restart the launcher, go back in and uncheck "Launch in Safe Mode" or it'll blot your settings again the next time you run the game.

If you are using the NCSoft Launcher, right-click on the City of Heroes entry and select Properties. On this page put a checkmark in the Launch in Safe Mode box and click OK, then launch the game.
If you are using the old original updater, start the updater, click Next but before clicking Accept on this second screen put a check mark in the Safe Mode box.
Something that concerns me is all of the Unknown entries in your CoH Helper report. I suspect that you didn't run CoH Helper as administrator and that it couldn't correctly report the registry settings due to this.
If you did run it as administrator, there are deeper issues in your computer.
As for the StarDock software, like HyperStrike I've seen several of the Stardock programs cause issues with the game, likely because they are using similar code in some areas and that code is part of what causes the problems.
If the game spit out 20 dollar bills people would complain that they weren't sequentially numbered. If they were sequentially numbered people would complain that they weren't random enough.

Ah yes, SafeMode confusion. I did actually put Windows into SafeMode, and not the game. This would explain my troubles with the drivers not loading and the game not loading because of the graphics drivers, or lack thereof.
I do have some interesting news to report. As I reported before, the game would freeze when I messed with the graphics settings, particularly FSAA. And unreported previously, (I think) I could not enter my SG Base without the game freezing and of course, it would freeze when I would exit the game. This happened with anything by Stardock on or off. So, thinking about the game SafeMode I decided to try that one more time. I had previously done this multiple times when GR came out, and while it was a way to reset some things, if I recall correctly, the game would even freeze in SafeMode.
Now before running in SafeMode, I had the game do a Repair (consistency check), but it found no problems. (this is another thing I had done in the previous months, only to have no problems found.) Only this time, instead of doing it from the CoH Launcher, I performed this check from the new NCSoft Launcher.
One more thing I did, that I just remembered. I went to NVidias site to check on new drivers. Im a bit obsessive about having the latest drivers. There was the one from last month, but also a new Beta Driver dated for today. I grabbed that thing and sat through the 10 minute install. But after all that, having my Stardock programs off, and willing to experiment a bit to solve my problems, I fired up a SafeMode CoH.
The first thing I noticed was the 800x600 resolution a resolution I have not willingly played at since 2005. That and the game was in Fullscreen Mode, not Windowed. But thats all right I can fix that. The Options menu opened, and I pulled up the Graphics tab with no troubles. First thing I do is turn the resolution back up to 1920x1200. Ah
.glorious HD widescreen! Then seeing as how if I were to mess something up, I can just run SafeMode again (or TweakCoH) and fix the settings, I start to turn up all the other stuff. Ultra Mode engaged. All the advanced settings turned on and up to 11. Everything looked good. I had shadows, bloom, wonderful shinies, all the good stuff. Then, feeling brave, I turned my attention to the FSAA that had most recently caused my frozen game state. It was already on 2x. Cautiously I hovered the mouse over the slider, and carefully pressed and held the button. Then I slid it to the right and 4x. Previously, the game would stop halfway between 2 and 3X, but this time nothing happened. No frigid game. Huh. I moved around, a bit , fired off a couple powers. Still stable. So I went to 8x FSAA. Still working. Feeling emboldened by this success, I pushed the slider further, 16x, then 32x! I didnt know the game or my card had a 32x FSAA setting! Still the game worked! Not wanting to jinx things, I quickly slid it back down to 8x. But I did go down and turn up the Antisotropic Filtering to 8x as well. The game was running beautifully.
So I logged to the character select and chose another Hero. This time I went straight to my SG Base. After about 10 seconds of trepidation, it loaded. I havent been in my base in months! And it loaded! No freezy stuff! Wow.
One last test to perform whist still in SafeMode. I quit the game to my desktop. This worked as well. No freezing. Amazing.
Thoroughly encouraged by my experience I restarted the game normally. It started like always, and I performed all the tests I had under SafeMode. All were passed with flying colors. I turned all the graphics options up and down and in various combinations and the game ran. I entered my SG Base and the game loaded it and ran. But I was still in Fullscreen mode. Growing suspicious of my luck so far, and thinking perhaps that the previous errors were some sort of weird conflict with the game and Windowed mode, I set the graphics for Windowed and logged out. This worked without any chilliness either. I cautiously restarted the game in windowed mode. It started, I logged in and braced myself for a sudden drop in programming temperature. I opened the graphics options, and did the same tests as before, sliding the sliders left and right with wild abandon, before finally settling on 11. Nothing happened. I flew to the Base Portal and entered my base. I was a bit worried when the loading bar didnt move, but up popped my base anyway. I then performed the exit test. The game smoothly quit to my desktop.
I performed the tests multiple times again. I have started my WindowBlinds and Deskscapes again, to see if that was the culprit. The game runs. Its running in the background as I type this. I can turn up or down all the graphics I want. I can enter my SG Base, and I can exit the game without the game going into not responding mode.
At this point, I have no idea if it was the Repair action, the SafeMode run, or the new Beta Drivers, or a combination thereof that has restored my beloved game to a pristine condition. I only wish that I could quantify for others what fixed it. Could it possibly be that I have had nine months of bad drivers from NVidia? I dont know. But my game runs like it should and I am quite satisfied.
I want to thank you folks for taking the time to look at my problem and offering suggestions on how to fix it. I really do appreciate the help.
Oh, one last thing.

I'm just happy I have a working game.

Second, I'm still puzzled by the Unknown entries in CoH Helper report. Another oddity I just noticed is that it also isn't reporting your Drives or the Free Space on them. I thought at first it might be due to Service Pack 1 so I ran CoH Helper myself and everything is correct in it's report. I'm even running the same version of Win 7 as you so I know it's not a problem there. I don't know if Zloth is still playing or not but I haven't seen him post in quite a while, so I doubt he'd be looking at the Tech Forums to see this himself.
Since CoH Helper only queries the registry for the game settings and then interprets those, I wonder if your Antivirus or some other program might be interfering with the queries CoH Helper runs.
As a final observation, I'm hesitant to try to use TweakCoH since Issue 17 was released. The last version of TweakCoH was released in March 2007, and none of the Ultra Mode settings had been implemented at that time. It's also possible that some of the older settings that TweakCoH could adjust have been changed slightly and the settings (registry entries) could get corrupted using TweakCoH now. I haven't had time to test TweakCoH since Issue 17 but I hope to get a chance to in the next month or so.
I'd love to see someone with some coding skills develop a new app similar to TweakCoH that can adjust the new graphics settings that are available, but since my programming experience is so outdated, I'd have to wait for someone else to do it.

As far as TweakCoH is concerned, after the launch of Going Rogue, it was the one tool that helped turn my settings down so I could actually play the game. As far as I can tell the devs of the game just added to their registry settings, not changed them entirely. For example, if FSAA previously had 2x, 4x, and 8x it would be listed in the registry as 0 (off), 1, 2, 3. With 16x and 32x, settings 4 and 5 were added. But Tweak was written when only 1, 2, and 3 were there, so it only ‘sees’ those values and allows you to change to those values. With the entirely new settings, TweakCoH doesn’t see them either, since it wasn’t written to see them, and therefore cannot change them. However, since there IS an overall settings slider from back when it was written, it does have an overall slider, it just doesn’t have an ‘ultra’ setting.
I do however, also wish that whomever wrote the TweakCoH program would be kind enough to update it. Or if that person is not willing, allow someone else who is willing and able to take the code and write an update this very helpful tool for us.

Ok. Gonna try this again. Ive been having problems with the game since Going Rogue came out. Or perhaps just before that with the advent of Ultra Mode graphics. I think it could be a graphics problem, but I have no idea. Before Ultra Mode came out the game ran fine. At that time I had a Geforce 8800GT card. I have since upgraded to a wonderful Geforce GTX470 card. The game RUNS fine, but there are some strange things happening.
Well start with Shadows. Whenever I even so much as try to turn on Shadows, the game freezes. First video freezes, then sound cuts out about ten seconds later. The same happens if I touch the Ambient Occlusion slider, Bloom Effects, or evern try to off the "Advanced Graphics Settings"option. I thought maybe if I could just turn that off and slide the generic graphics slider on up to Ultra, it would help, but no-go on that either.
When GR first came out, I couldn't run the game at all without it freezing within a minute. TweakCoH fixed that problem, but now, even with my new, improved, shiny card, I can't get the game to look as good as it can. It's very frustrating.
TweakCoH lets me adjust my shader quality to high, but when I get in-game my shadows are disabled. I've updated my graphics drivers religously over the past six years. I do wish that TweakCoH could be updated to reflect the new settings within the game, as this would most likely help me track down where the issue is.
Since I first reported my problems back in February (and got no real help, sadly), I have uninstalled my graphics drivers, restarted the machine, reinstalled the drivers, and no help.
I have fiddled with the settings in TweakHelper, turning some things down, but now the game freezes if I even click the Graphics Options tab! I deleted the game and reinstalled it. Same problem.
To get the pretty graphics, I resorted to manually editing my registry. Carefully. Since then I can get the game to run and it looks wonderful.
However I still have the problem that if I even so much as touch the mouse to the Graphics Option in the Options tab, the game freezes. This makes it really hard to adjust anything for new characters such as chat bubbles.
I also cannot enter my SG base with the graphics turned up. At all. The game freezes with about 10% of the base loaded.
I also have one more problem with the game freezing. This other problem is that I cannot exit the game in any normal way. I play in windowed mode, and have for almost seven years. Never have I had this problem. If I hit the exit X button on the windows the game freezes. If I Quit to Desktop it happens. I can log out to either the character select, or log on screens just fine. But as soon as I hit that Exit button the game freezes.
I cannot help but think that all these freezings of my copy of the game are related. They all started with the release of Going Rogue. Something in my files has changed, and not for the better.
As I said I can RUN the game, and when I do it looks wonderful. But not being able to enter my SG base, touch the Graphics settings or exit the game without the whole thing freezing kinda takes away some of the reason I play.
Help would be much appreciated. Again.

Nothing in either report jumps out to me as being bad or wrong. But I shall let other, more experienced eyes have a look. Yes I have a lot of things open in the background. Typically IE9 with multiple windows, Outlook, Firefox, Chrome and Messenger. Sometimes Word. Then again I have 12 Gigabytes of memory, and no matter how much I have open, I always have enough to run the game. I have run the game with multiple programs open going from a single core, to dual core, to my newest four-core cpu and anywhere form 2 to my now 12 gigabytes of RAM. Never before GR has this game had any problems playing or freezing. Even when I have tried the training room.
