I think I18 removed it from our trays because mine disappeared too.
The reason I suspect I18 is side switching probably brought about an issue with the portals since heroes and villains go to different Ouroboros.
I know there is a bug with the black market teleporter in the base after a respec.

Oddly it's only on raised floors that it fails(and only Red-side bases).
