Trick or Treating not working at all.

If I'm not mistaken, you need to be in a level-appropriate zone for ToT to work.
For instance, a level 50 toon in Atlas won't get ToT doors, but will in Peregrine Island.
Could that be the issue?
We don' need no stinkin' signatures!

Zones Trick or Treating only works if the player was in a zone with a level appropriate for their current combat level. Also make sure you don't have a mission selected (with the red directional marker).
You may need to complete one or abandon it

Nerva doesn't seem to go all the way to 40th. My level 38 stalker was unable to ToT there this morning. I popped around the main islands but never found a door that would open.
Luckily St. Martial worked fine. I alternated between the 4 double doors near Vanguard and made a killing.
