Is it possible you missed some of his friends? I think you have to kill the hero and the support team to get credit.

It happens sometimes that when you get defeated one or more of the Longbow decide to chase you down at the police station. I found one in one Mayhem mission standing 150 yards from the bank, just twiddling his thumbs because he lost track of where I was to come attack me.
I chalk it up to the AI just doing stupid things occasionally. Sometimes a couple of the Longbow will run off, forcing you to chase it down to complete the mission.

As mentined, you have to kill the whole mob that comes with the Hero. If anybody on the team is not in the Bank, they might go to a different location and then you've pretty much got a "Defeat All" on your hands until you find the right one... unless you are good at finding out where he went. You mentioned you died a few times, so when you went back to the Police station, the other foes you needed could have gone anywhere. Usually if you are solo and stay in one place long enough, they will find you, but it's not always the case. The best thing you can do in that scenario is just keep killing guys and get all the extra time you can to find the ones you missed.
As for the bonuses you saw for payphones, if you mean that you were standing around doing nothing and you got the extra time, it's possible that some of the other enemies in the area defeated them and gave you the extra time.

theres also one bank mish in which i didnt get a mish complete and then went outside and saw a HORDE of flying longbow like 150yds above the bank and apparently all of them were part of the ambush
i know there is a glitch with bank mishs in which longbow eagles can fly through the bank walls and such
