A couple other people have reported having issues with her spawning. Try resetting the mission. From what I can tell, that's the only fix, and it might take a few resets.
Make sure to /bug it, so that they can get it fixed.

In one case she was in the main room, in another case she was not too far from the mission entrance.
Edit: Also, by some reason she was -2 (level 48) in second mission.

Just to back this up - there've been a couple of threads about his recently - one had hard data on a work-around.
Try searching "Black Swan" here in the Player Questions forum section, see what pops up.
