Depending on your hardware...
Looking up and down is right-mouse and rotating around your character is the wheel.
My MBP has a large touch-pad with no "button" perse, but in System Preferences, you can activate the right-click 'corner'... which makes the bottom-right corner of the touch-pad a surrogate right-click.
My suggestion would be to purchase a simple usb/bluetooth 3-button scroll-mouse. It makes the gaming experience.... *pleasurable*...
