Have you got a mouse utility running? Sometimes those will override CoH. When I got a Logitech mouse, I had to disable all the special button functionality so that CoH could read buttons 4 thru 6.

I am having this issue with new logitech mouse.
I removed the Logitech mouse driver and used the default in Windows.

/unbind mousewheel2. Outside the game try these actions:
/bind MOUSEWHEEL "+camdistadjust"
A. Reboot
B. Delete your mouse driver and let windows install default.
C. Go to website of mouse manufacturer for an updated driver.

This thing been bugging me for quite awhile. I found that the problem is the accelerated scrolling speeds some mice have.
A workaround for me was installing a program called X-Mouse Button Control, and changed it so it intercepts scroll up and down with normal scroll up and down. Fixed it for me, in all games.
I really don't miss the accelerated scroll feature at all 
Had this happen a couple of years back when I installed a Microsoft Explorer mouse - no matter what I did I couldn't get the scroll wheel to zoom.
Eventually I got the zoom back - after I changed the mouse! - seems to be a problem with the mouse driver - and yes I had tried updating to the latest version.

That's what I figured the solution would be. How I hate computers!
I mean when they work they are awesome, but that's how they get us emotionally involved before twisiting the soul-wrenching knife of complexity.
I tinkered with some setting and by selecting generic for buttons three through five I get functionality back.
