Ambushed in Hidden Mode?
Simply put - the game cheats...
The longer explanation is that there are two kinds of ambush; positional and
targeted.
Positional: The mobs go to the spot you were at when they triggered. If you
move from there (or are hidden), they arrive and twiddle their thumbs unless
you attack... There are a number of these ambushes scattered throughout
(esp Arachnos missions).
Targeted: These guys (for whatever reason you want to imagine to justify it)
know exactly where you are, and they're out to bust a cap in your butt.
Mayhems, Bank Robberies, and various Story-Arc missions have these. You
simply have to deal with them...
My stalkers have caltrops, so my usual approach is to set myself up in a
target funnel or choke point, lay down the trops and chuckle in amusement
as they decide how they're going to get to me... While that is going on,
I usually tp-foe the nastiest one to me, kill him, and then deal with the
riff-raff after that...
As far as adjusting your hide (ie. with a stealth power or proc), it won't
matter for targeted ambushes. As for positional, it would only help if there
are mobs having perception better than simple hide, but less than capped
stealth. I can't recall if there are any such, but I'm sure someone will
chime in if there are. In PvE, hide by itself covers pretty much everyone
except snipers, drones, Rularuu Eyeballs, etc. All of those (I think) also
see through capped stealth, so it mostly won't change anything. Capped
stealth is really only vital for PvP.

Was curious about this myself a while back. Snipers actually have enhanced perception (they can see regular players from farther distances).
Not everything with enhanced perception automatically perceives you through stealth powers. However, most things with it will see you before (or just as) you reach melee range with them, making it a moot distinction for a lot of Stalker powersets.
The one true distinction is Rikti drones. They completely ignore stealth. They cannot actually see further than normal, and will not aggro at increased range (even for unhidden characters).

Keep in mind too that "Hidden" does not equal "I have no aggro". Not sure how or if ambushes relate to (or generate?) aggro but its quite possible to be "Hidden" and have mobs make a beeline for you because you have generated aggro somehow. My Claws/Nin learned early to remember if and where her caltrops were as I've had more than a few mobs chase me around several corners after a patrol or returning runner wanders into my caltrops still laying about from a fight.
This question has been on my mind since I started up my Stalker who just reached 50 . When entering some missions where your goal is to either take out a particular Boss, EB or AV, I sometimes got ambushed when I get close to the Boss. Case in point, when doing some mish'es in Cimerora, I will usually get ambushed by gladiators prior to enter the area where the Boss is.
So, with that being said, is there some kind of code within the game that says when you enter a zone that has a goal of "Defeating So and So" that you will normally get ambushed regardless if you are in Hidden mode or not? Keep in mind, I do nothing to provoke these ambushes but they can see me as if I stirred up a hornets nest. It really wasn't a problem as I could easily handle the rush of foes coming my way.
As my understanding on how "Hidden" works, you shouldn't be seen at all except by the Rikti Drones or maybe some very high level Bosses. I do know that in some mish'es to where you are to click on a "blinky" and then they run over to see who is there, I could stand right next to the blinky and they would not see me in Hidden mode. I have read somewhere that you "can" combine other stealth powers with "Hidden" to give you even greater invisibility. Not sure if this would effect the occasional ambush you get as I mentioned above?
