Mayhem explorations bugged?

I noticed that some of my toons are missing some of the mayhem exploration badges. At first I was not sure if I had just missed a mayhem or two with some characters, but checking other toons I realized that this problem extends to virtually all of my toons that do not have the accolade yet, even those with which I am 100% positive to have collected the badges in question. Especially "Road Rage" and "Tyrannical" seem to be frequent candidates for getting lost in the deep space of data bank errors.
After asking around a bit I found out that I am not the only one with this problem, so I thought I might just ask if there is a memo about this bug I have missed. Was this a one time issue where all those badges became lost or can it happen again at any time? Since there is an accolade tied to those exploration badges I am not too fond of the thought of having to exemp down an indefinite amount of times until the badges finally stick.

That's quite strange as i've recently done all mayhems up to talos island bank and received the exploration badges each time...

It is not that I did not receive them. The problem is that at some point in the past I have lost them.
Also, another strange point I and a friend who has the same problem noticed: When you do a mayhem mission again to get such a lost badge back there is no "Badge Earned" message on the screen. There is just the message in the chat window and the badge reappears in you list.
