Is this using standard SO's or normal IO's?
Anyway things I notice are the damage in Divine Avalanche - not something Iv'e ever tried with mine but seems a little wasted.
Health and Stamina should be fine with two slots if using IO's.
Dodge at level 32 instead of Golden Dragonfly is not something I'd advise since this is your best attack and the defence gained from Dodge can wait for now. Take the attack and get it slotted ASAP.
While I've never had any /SR characters above level 10, I'd assume most people would select Evasion come level 35?
Level 38 should be the time to pick the 'God' mode power 
At 41 that would leave Dodge or a patron power - if your going Mako then I'd highly recommend the fun Water Spout that can help against foes you fear most/semi tank/panic button. Two or three recharge tops for that - add possibly an endurance reduction as its alot to summon. I'm still unsure about defence debuffs, damage and stun durations on it, maybe someone has numbers for those?
The hold looks good but drop that range for another recharge.
Seems Dodge for me would appear at 47 if your looking to take the team friendly Recall Friend at 49.
Expect some folk to suggest taking Aid Self which will no doubt help but that on such a tight build with these sets loses the fun factor of Mako's powers imho
Whatever you do just don't skip that tier9 attack at level 32 
Oh and if your on Union, then look for 'Uzzi' who I consider to be a top notch Stalker and has the same sets as you (he's running a now uber IO +def/+recharge/+max HP/+dmg build costing him 'a fair bit').
Uzzi FTW! He's seriously one of the best stalkers out there.

Recall friend was the first thing that came to my mind because it only needs the inherent slot to work. And since he's a PvE build maybe I can skip the aid self.
I'm based on Defiant so unless Uzzi browses the forums, no chance of meeting him.

I've just come across this topic by chance! Not often I see a Ninja Blade/SR topic around, cheers for the compliment fellas
It's a decent build which will serve you fine but there are some slotting which you might need to take into consideration.
Slotting recharge on Sting of the Wasp and Flashing Steel, it may become redundent with later levels as you will have other attacks which you can use to fill in. Since the recharge on them is quite low and you are already 30 (with Quickness), it may be worth saving a slot or using the 6th slot to put an Endurence Reduction in them instead.
Focused Fighting/Senses, would be worth putting in 3 defences in there as this is your main defence so it needs to be as high as possible. You could get away with Focused Fighting as you are using Divine Avalanche but that's down to personal preference. Wouldn't worry about Evasion's defence slotting too much as hide gives a ridiculous amount of defence in AoE.
Divine Avalanche, not sure if damage is worth putting into this power but very good filler and good call on putting defences in there.
I would max out the damage on Golden Dragonfly which may mean removing a recharge or endurence reduction, can play it by ear on which will be the better choice.
You wouldn't need an Endurence modification on Elude, the recovery is great on that power unslotted. Would recommend replacing for an extra defence.
The Leviathan powers I would not have any experience in, best to take MajikMan's advice on that.
Super Reflexes is difficult for a stalker in terms of power choices. Reason being is that it's usually desired to take all of them which I did and thus sacrificed all of my Patron powers.
Some other Power Pools to consider:
Concealment - You may be thinking that stealth is only really needed for PvP but I find it surprising that so few stalkers do not use it for PvE (especially defenced based sets) it adds a bonus to defence and definately makes a difference in fights. The only problem here is the massive endurence cost to it so you would have to find some slots to keep this power going.
Medicine - Again down to personal preference but many do like that self heal added on top of defence. You'd need to use up a power before you get this though and then slotting will come into play. I never took it as I just could not see it fitting character concept (a Samurai/Ninja running around with a handy tricorder) and got along fine, it isn't mandatory for PvE.
Speed - Hasten's generally a good thing on most builds and it does not disappoint on this one. Worth it for the extra recharge on Assassin's Blade and Placate so you can get in those signature attacks in more often, as well a nice recharge on you other attacks.
Overall good stuff, there's also some powers which I would shuffle around but as you're 30 it wouldn't make a difference unless you get malefactored.

Now many of choices of slotting in there is debatable and should be tweaked to your own preference. Believe it or not it took me a good 10 minutes to think of what to put down as the first 30 powers and reluctently put Stamina in at 28. Personally I sacrificed Flashing Steel and that made the power choice a lot easier. Here is what I got without Flashing Steel (didn't slot this one):

The only many thing that you should be aware of was moving the travel powers to 22/24. This was delibarate since, as a stalker moving around is quite easy and even over a year I still have my temp travel powers for when I do get malefactored down.
